Indications/Uses
Rapid conversion to normal sinus rhythm of paroxysmal supraventricular tachycardias, including those associated w/ accessory by-pass tracts (Wolff-Parkinson-White syndrome) in adults. Aid to diagnosis of broad or narrow complex supraventricular tachycardias. Sensitization of intra-cavitary electrophysiological investigations.
Dosage/Direction for Use
IV Administer via rapid IV bolus inj. Adult including elderly Initially 3 mg given over 2 sec. Give 6 mg as 2nd dose or 12 mg as 3rd dose if previous dose does not result in elimination of supraventricular tachycardia w/in 1-2 min.
Contraindications
Hypersensitivity. Patients suffering from 2nd or 3rd degree AV block & sick sinus syndrome (except in patients w/ functioning artificial pacemaker); chronic obstructive lung disease w/ evidence of bronchospasm (eg, asthma); long QT syndrome; severe hypotension; decompensated states of heart failure.
Special Precautions
Discontinue use if angina, severe bradycardia & hypotension, potentially fatal resp failure or asystole/cardiac arrest occurs. Severe bradycardia in early post heart transplant patients or those w/ occult SA disease. Increased heart sensitivity in patients w/ recent heart transplantation (<1 yr). Possibility of transient cardiac arrhythmias arising during conversion of supraventricular tachycardia to normal sinus rhythm. Increased conduction down the anomalous pathway in patients w/ atrial fibrillation/flutter & accessory by-pass tract. May precipitate or aggravate bronchospasm in patients w/ COPD. Patients w/ left main coronary stenosis, uncorrected hypovolemia, stenotic valvular heart disease, left to right shunt, pericarditis or pericardial effusion, autonomic dysfunction or stenotic carotid artery disease w/ cerebrovascular insufficiency; recent MI, heart failure, or minor conduction defects (1st degree AV & bundle branch block) that could be transiently aggravated during infusion; congenital, drug induced or metabolic prolonged QT interval. Monitor patients w/ history of convulsions/seizures. Not to be administered in patients receiving dipyridamole. Not to be used during pregnancy & lactation. Childn 0-18 yr.
Adverse Reactions
Bradycardia, AV block, sinus pause, skipped beats, atrial extrasystoles, ventricular excitability disorders eg, ventricular extrasystoles, non-sustained ventricular tachycardia; dyspnea; flushing; chest pressure/pain, feeling of thoracic constriction/oppression. Headache, dizziness/light headedness; nausea; burning sensation; apprehension.
Drug Interactions
Potentiated action w/ dipyridamole. Avoid concomitant use w/ aminophylline, theophylline & other xanthines for 24 hr prior to use. Avoid food & drinks containing xanthines (tea, coffee, chocolate & cola) for 12 hr prior to use. Concomitant use w/ drugs tending to impair cardiac conduction.
