Furetic/Furetic-S

Edema associated w/ CHF, renal insufficiency or hepatic cirrhosis. Furetic tab: HTN. Furetic inj/Furetic-S: Adjunctive therapy in hypertensive crises & pulmonary edema.

Furetic tab Adult Edema 20-80 mg daily as single dose. May be increased by 20 or 40 mg increments no sooner than 6-8 hr after previous dose (as once daily or bid) & titrate up to 600 mg daily in patients w/ severe edema. HTN 40 mg bid. Childn Edema Initially 2 mg/kg as single dose & titrate up to 1-2 mg/kg every 6-8 hr to max dose of 6 mg/kg. Usual dose: 4 mg/kg once daily or bid. HTN Initially 0.5-2 mg/kg once daily or bid. Max: 6 mg/kg daily. Renal impairment Initially 80 mg daily. May be increased up to 80-120 mg daily. 320-400 mg daily when immediate diuresis is needed. Chronic renal failure Initially 250 mg, may be increased in steps of 250 mg every 4-6 hr. May titrate dose up to 1,500 mg/24 hr. Furetic inj/Furetic-S IV dose administered via slow infusion over 1-2 min. Adult Edema Initially 20-40 mg IM or IV. May be increased in 20 mg increments given not more than every 2 hr. Hypertensive crisis 40-80 mg IV w/ other antihypertensives. Pulmonary edema Initially 40 mg IV. May be increased to 80 mg IV if desired effect is not achieved w/in 1 hr. Childn Initially 1 mg/kg slow IM or IV, may be increased by 1 mg/kg at 6-12 hr intervals. Max: 6 mg/kg. Renal impairment Initially 100 mg to 2 g at 2-24 hr intervals. Max: 6 g IV daily.

May be taken with or without food: May be taken w/ meals to reduce GI discomfort.

History of hypersensitivity. Anuria.

Sulfonamide sensitivity. Dehydration & blood vol reduction w/ circulatory collapse & possibly vascular thrombosis & embolism, especially in elderly; electrolyte imbalance (eg, hyponatremia, hypokalemia, hypomagnesemia, hypocalcemia, hypochloremic alkalosis) in long-term use. May cause hyperuricemia exacerbation; hearing impairment (eg, tinnitus) & ototoxicity. Perform periodic determinations of serum electrolytes (eg, K, Ca, Mg) for signs of fluid or electrolyte imbalance. Hepatic cirrhosis. Pregnancy. Discontinue breastfeeding during therapy. Furetic inj: Avoid use in case of hypersensitivity. Discontinue treatment if rash & flu-like syndrome; SJS (fever, red rash, skin blisters, skin peels away in sheets & mucous membranes inflame eg, oral throat, nose, reproductive organs & eyes); methemoglobinemia (bluish skin color & weakness) occur. Patients w/ G6PD deficiency. Hepatic & renal impairment. Avoid use in pregnancy (3rd trimester) & lactation due to hyperbilirubinemia in infant. Avoid use in childn <2 yr.

Anorexia, nausea, vomiting, diarrhea, abdominal cramp, pancreatitis, jaundice; headache, dizziness, blurred vision, hearing loss or impairment, weakness; anemia, leucopenia, thrombocytopenia; photosensitivity, urticaria, rash; orthostatic hypotension; muscle & urinary bladder spasm, weakness, fever, hyperuricemia, hyperglycemia, urinary frequency. Furetic inj: Irritation & pain at inj site.

Interference of hypoglycemic effect w/ insulin or antidiabetics eg, sulfonylureas. May potentiate hypotensive effect w/ other antihypertensives. Electrolyte abnormalities w/ thiazide diuretics. K depletion & cardiac arrhythmias w/ digitalis. Decreased K level w/ glucocorticoid, corticosteroid, corticotrophin & amphotericin B. Increased ototoxicity w/ aminoglycosides. Reduced efficacy w/ phenytoin or indomethacin, & other NSAIDs. Reduced clearance of lithium. Increased plasma level of propranolol.
