Lidocaine HCl
Indications/Uses
Relief of irritated or inflamed mucous membrane of mouth & pharynx. Reduces gagging during X-ray picture taking & dental impressions.
Dosage/Direction for Use
Adult Mouth anaesth 15 mL swished in the mouth & spit out, at intervals of no <3 hr. Max dose: 4.5 mg/kg or 300 mg/dose (8 doses/24 hr). Pharynx anaesth 15 mL gargled at intervals of no <3 hr. May be swallowed if necessary. Max dose: 4.5 mg/kg or 300 mg/dose (8 doses/24 hr). Adolescent & childn ≥3 yr Not to exceed 4.5 mg/kg or 300 mg/dose, swished in the mouth & spit out, at intervals of no <3 hr. Max dose: 4 doses/12 hr. Childn <3 yr & infant ≤1.2 mL applied to area, at intervals of no <3 hr. Max dose: 4 doses/12 hr. Use only if underlying condition requires treatment w/ product vol ≤1.2 mL.
Contraindications
Hypersensitivity to lidocaine HCl or another local anaesth of amide type; methyl-/propyl paraben or PABA metabolite.
Special Precautions
Not to be used in teething pain. Not to be given to patients w/ hypovolemia, heart block or other conduction disturbances. May impair swallowing & increase risk of aspiration when used in mouth or throat. Patients w/ CHF, bradycardia, or resp depression. Do not eat or drink for at least 3-4 hr after receiving local anaesth for procedures eg, laryngoscopy or tracheoscopy. Renal & hepatic impairment. Pregnancy & lactation. Not approved for relief of teething pain & discomfort in infants & childn. Childn <3 yr.
Adverse Reactions
Restlessness, excitement, nervousness, paraesthesias, dizziness, tinnitus, blurred vision, nausea & vomiting, numbness of tongue, lightheadedness followed by sedation, excitation (transient or followed by depression w/ drowsiness, resp failure & coma); hypotension, bradycardia, arrhythmias & possible cardiac arrest; allergic reactions (cutaneous lesions, urticaria, edema or anaphylactoid reaction).
Drug Interactions
Increased toxicity w/ antiarrhythmics (eg, mexiletine). Enhanced metabolism w/ antiepileptics (eg, phenytoin or barbiturates). Additive cardiac depressant effects w/ phenytoin. Increased plasma conc w/ β-blockers (eg, propranolol, nadolol & metoprolol). Reduced hepatic metabolism & decreased hepatic blood flow w/ H2-antagonists (eg, cimetidine).
