Indications/Uses
For susceptible infections including bone & joint infections, bronchitis (& other lower resp tract infections), gonorrhea, meningitis, otitis media, peritonitis, pharyngitis, sinusitis, skin infections (including soft tissue infections), & UTI. Surgical infection prophylaxis.
Dosage/Direction for Use
Kefsyn FC tab UTI 125 mg bid. Resp tract infections 250-500 mg bid. Adult Pneumonia or acute exacerbations of chronic bronchitis 750 mg-1.5 g bid via IV route followed by 500 mg bid orally in each case. Lyme disease 500 mg bid for 20 days. Uncomplicated gonorrhea 1 g as single dose, may be given w/ probenecid 1 g. Childn >2 yr Otitis media 250 mg bid or 15 mg/kg bid. Max: 500 mg daily. Childn >3 mth 125 mg bid or 10 mg/kg bid. Max: 250 mg daily. Kefsyn 750 IV/IM Adult 750 mg tid, may be increased to 1.5 g tid in severe infections. The frequency of administration can be increased to 6-hrly if necessary, giving total doses of 3-6 g daily. Infant & childn 30-60 mg/kg/day increased to 100 mg/kg daily if necessary, given as 3-4 divided doses. Impaired renal function CrCl <20 mL/min 750 mg bid, 10 mL/min 750 mg once daily. Patient on hemodialysis 750 mg dose should be given at the end of each dialysis. Continuous peritoneal dialysis 750 mg bid.
Administration
Kefsyn FC tab May be taken with or without food.
Contraindications
Hypersensitivity to cefuroxime, cephalosporin or any other β-lactam antibacterial agent eg, penicillins, monobactams & carbapenems.
Special Precautions
Discontinue use in case of severe hypersensitivity reactions. Patients who have experienced allergic reaction to penicillins or other β-lactam antibiotics; history of non-severe hypersensitivity to other β-lactam agents. Use may result in overgrowth of Candida. Prolonged use may also result in overgrowth of other non-susceptible microorganisms eg, enterococci & Clostridium difficile. Pseudomembranous colitis. Monitor renal & haematological status especially during prolonged & high dose therapy. Patients w/ renal impairment. Pregnancy & lactation. Kefsyn Jarisch-Herxheimer reaction following treatment of Lyme disease. Development of +ve Coombs test may interfere w/ cross matching of blood. False -ve result may occur in ferricyanide test. Kefsyn 750 Concurrent use w/ diuretics. Safety & effectiveness have not been established in childn <3 mth.
Adverse Reactions
GI disturbances, including diarrhea, nausea, & vomiting. Kefsyn 750 Transient pain at the site of IM inj; skin rashes (maculopapular & urticaria), drug fever; pseudomembranous colitis; decreased Hb conc &/or eosinophilia, leukopenia & neutropenia; transient rise in SGOT & SGPT, & bilirubin.
Drug Interactions
Kefsyn Reduced renal clearance w/ probenecid. Kefsyn 750 Concurrent use w/ potent diuretics eg, furosemide & aminoglycosides may adversely affect renal function.
