Pregnancy: In cases of paracervical block the risk of reactions such as bradycardia/tachycardia in the foetus increases, therefore foetal heart rate must be closely monitored.
It should also be noted that the addition of adrenaline can reduce uterine perfusion and uterine contractions during parturition, especially in cases of intravenous injection. See also Pharmacology: Pharmacokinetics under Actions.
Lactation: Bupivacaine passes into breast milk, but the risk of this affecting the child appears unlikely with therapeutic doses. It is not known whether adrenaline passes into breast milk, but the risk of an effect on the child appears unlikely with therapeutic doses.
