Ivabradine HCl
Indications/Uses
Symptomatic stable angina pectoris in adults whose heart rate is ≥70 bpm & who do not tolerate or cannot take β-blockers, or in combination w/ β-blockers in adults whose condition is not fully controlled w/ a β-blocker. Chronic heart failure in adults whose heart rate is ≥75 bpm in combination w/ standard therapy, including β-blocker therapy, or when β-blockers are contraindicated or not tolerated.
Dosage/Direction for Use
Patient <75 yr Initially 5 mg bd, may be increased to 7.5 mg bd (1 tab in the morning & 1 tab in the evening), ≥75 yr 2.5 mg bd (½ of 5-mg tab in the morning & ½ of 5-mg tab in the evening).
Administration
Should be taken with food.
Contraindications
Hypersensitivity. Concomitant use w/ ketoconazole, itraconazole, josamycin, clarithromycin, telithromycin, oral erythromycin, nelfinavir, ritonavir, nefazodone, verapamil, diltiazem. Patients w/ resting heart rate <70 bpm before treatment; cardiogenic shock; sick sinus syndrome, SA block, 3rd degree AV block; heart attack; very low BP; unstable angina; worsening heart failure; heartbeat exclusively imposed by pacemaker; severe liver problems. Women of childbearing potential not using reliable contraception. Pregnancy & lactation.
Special Precautions
Discontinue treatment if there is no improvement in symptoms of angina w/in 3 mth after treatment initiation. Consider treatment discontinuation if there is only limited symptomatic response & when there is no clinically relevant reduction in resting heart rate w/in 3 mth. Caution in patients w/ heart rhythm disorders or sustained atrial fibrillation, or long QT syndrome; symptoms of heart slowing down too much eg, tiredness, dizziness, shortness of breath; symptoms of atrial fibrillation (pulse rate at rest >110 bpm or irregular); recent stroke; mild to moderate low BP; uncontrolled BP; severe heart failure or heart failure w/ bundle branch block; chronic eye retinal disease; moderate liver problems; severe renal problems. Avoid intake of grapefruit juice. Contains lactose; caution in patients w/ intolerance to some sugars. May cause temporary luminous visual phenomena; caution when driving or using machines. Do not give to childn & adolescents <18 yr.
Adverse Reactions
Luminous visual phenomena. Slowing down of heart rate, atrial fibrillation, bradycardia, ventricular extrasystoles, 1st degree AV block, uncontrolled BP, headache, dizziness, blurred vision.
Drug Interactions
Potential interaction w/ fluconazole, rifampicin, barbiturates, phenytoin, St. John's wort, quinidine, disopyramide, ibutilide, sotalol, amiodarone, bepridil, pimozide, ziprasidone, sertindole, mefloquine, halofantrine, IV erythromycin, pentamidine, cisapride, furosemide, hydrochlorothiazide, indapamide. Increased exposure w/ grapefruit juice.
