Indications/Uses
Dosage/Direction for Use
Adult Mild hyperthyroidism Initially 15 mg once daily in 3 divided doses. Moderate to severe hyperthyroidism 30-40 mg once daily in 3 divided doses. Severe hyperthyroidism 60 mg once daily in 3 divided doses. Total daily dose: Usually given in 3 equally divided doses at approx 8-hr interval. Maintenance dose: 5-15 mg once daily. May be given as single daily dose in many cases. Childn Initially 0.4 mg/kg once daily in 3 equally divided doses at approx 8-hr interval. Maintenance dose: 0.2 mg/kg once daily in 3 equally divided doses at approx 8-hr interval.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Risk to cross-sensitivity when switching from 1 thioamide agent to other. Not recommended to use alternative antithyroid drug ie, propylthiouracil.
Special Precautions
Discontinue treatment if pruritic rash, jaundice, alcoholic stools, dark urine, arthralgias, abdominal pain or fatigue occurs; in the presence of hepatitis (transminase >3x ULN). Monitor for signs or symptoms of illness eg, sore throat, skin eruptions, fever, chills, headache or general malaise. Agranulocytosis; aplastic anemia, thrombocytopenia & leukopenia; hypoprothrombinemia & bleeding; hypothyroidism. Patients receiving other drugs causing myelosuppression (particularly agranulocytosis) & in patients >40 yr. Avoid doses ≥40 mg daily. Perform baseline CBC including white count differential; LFT including alkaline phosphatase, aminotransferase & bilirubin prior to therapy. Monitor prothrombin time during therapy, especially before surgical procedures; thyroid function every 1-3 mth for 6-12 mth after discontinuation & if symptoms of hyperthyroidism occur. Pregnancy & lactation.
Adverse Reactions
ANCA-positive vasculitis, edema; drowsiness, fever, headache, neuritis, vertigo; alopecia, exfoliative dermatitis, pruritus, skin pigmentation, skin rash, urticarial; goiter, hypoglycemic coma; constipation, epigastric distress, loss of taste perception, nausea, salivary gland swelling, vomiting, wt gain; agranulocytosis, aplastic anemia, granulocytopenia, hypoprothrombinemia, leukopenia, thrombocytopenia; hepatic necrosis, hepatitis, jaundice; arthralgia, myalgia, paresthesia; nephritis, insulin autoimmune syndrome, lympadenopathy, SLE-like syndrome.
Drug Interactions
Increased levels/effects of aripiprazole, cardiac glycosides, clozapine, lomitapide, pimozide, theophylline derivatives. Decreased levels/effects of Na iodide, vit K antagonists.
