Pharmacology: Pharmacodynamics: Carbocisteine reduce the goblet cell hyperplasia. Another study showed that effects of carbocisteine on fucosylated and sialylated sugar chains on airway mucin (MUC5AC) may partially explain increased viscosity of mucous.
Anti-inflammatory properties via modulation of fucose residues on alveolar macrophages leading to increased clearance of apoptotic cell has also been described.
Pharmacokinetics: Absorption: Carbocisteine is rapidly and well absorbed from the gastrointestinal tract and peak plasma concentrations occur approximately 1 to 3 hours after oral dose.
Distribution: The volume of distribution (Vd) is about 60 to 105 L and it distributes to lung and respiratory.
Metabolism: Multiple metabolic pathways leading to multiple metabolites may be involved, including glucuronidation, N-acetylation and sulfoxidation. Metabolism may vary due to circadian rhythm and genetic polymorphisms.
Excretion: Urine (primary route, 15.6% to 37.9% recovered as unchanged drug after 16 hours).
Mucolytic: Adjunctive mucolytic therapy in respiratory tract disorders characterized by excessive or viscous mucous secretions.
Recommended dose: Adults and children more than 12 years: 1 capsule 3 times a day.
Mode of Administration: Carbocisteine is given orally and should be taken with food.
Overdosage and treatment: Gastric lavage may be beneficial, followed by observation. Gastrointestinal disturbance is the most likely symptom of carbocisteine overdose.
Hypersensitivity to carbocisteine or any component of the formulation.
Active peptic or duodenal ulcer.
Gastrointestinal: Gastrointestinal bleeding has been rarely reported; use in patients with active peptic or duodenal ulcers is contraindicated in some product labels.
Respiratory: Use with caution in patients with chronic bronchial asthma or respiratory failure. Paradoxical respiratory adverse effects including increased bronchial secretions leading to hospitalization have been reported in infants and children from 2.5 to 34 months; effects may be related to decreased spontaneous drainage in this population.
Effects on ability to drive and use machines: Carbocisteine has no or negligible influence on the ability to drive and use machines.
Use in Children: Use is contraindicated in infants and children <2 years. Capsules dosage form is not recommended for use in children <12 years.
Pregnancy: There are no available data on carbocisteine use in pregnant women. No conclusions can be drawn regarding whether or not carbocisteine is safe for use during pregnancy. The use of carbocisteine in pregnant women is not recommended, especially during the first trimester.
Breastfeeding Patients: There are no available data on the presence of carbocisteine in human milk, milk production, or the effects on the breastfed infant. No conclusions can be drawn regarding whether or not carbocisteine is safe for use during breastfeeding. The use of carbocisteine in breastfeeding women is not recommended.
Incidence 1-10%: Gastrointestinal: Diarrhea, nausea and stomach discomfort.
Incidence <1%: Abdominal pain, allergic skin reaction, angioedema, anorexia, bullous dermatitis, dizziness, erythema multiforme, headache, skin rash and Stevens-Johnson Syndrome.
Frequency not defined: Cardiovascular: Palpitations.
Dermatologic: Erythema, pruritus, urticaria.
Endocrine & metabolic: Hypoglycemia (mild).
Gastrointestinal: Gastrointestinal hemorrhage, vomiting.
Hypersensitivity: Anaphylaxis, fixed drug eruption.
Avoid concomitant use of Carbocisteine with Disulfiram and Metronidazole (systemic).
The level/effects of Carbocisteine may be increased by Cefotetan, Chloramphenicol (systemic), Disulfiram, Griseofulvin, Ketoconazole (systemic), Metronidazole (systemic), Procarbazine and Sulfonylureas.
Store CARNEX in a dry place at temperature not exceeding 30°C.
