Indications/Uses
Management of pain when use of opioid analgesic is appropriate. Pre-op or preanesth medication, supplement to balanced anesth, & for relief of pain during labor.
Dosage/Direction for Use
Pain relief IV 1 mg single dose every 3-4 hr as needed. Effective dose range: 0.5-2 mg, depending on pain severity, every 3-4 hr. IM 2 mg single dose, particularly in patients who can remain recumbent in case of drowsiness or dizziness, may be repeated every 3-4 hr as necessary. Effective dose range: 1-4 mg repeated every 3-4 hr. Use as pre-op/pre-anesth medication IM Individualized dose. Adult 2 mg administered 60-90 min before surgery. Use in balanced anesth IV 2 mg administered shortly before induction. 0.5-1 mg may be given in increments during anesth. Doses can be increased up to 0.06 mg/kg (approx 4 mg/70 kg), depending on prior administration of sedatives, analgesics, or hypnotics. Total dose range: 4-12.5 mg (0.06-0.18 mg/kg), depending on individual requirements. Use in labor Full-term patient in early labor 1-2 mg IV/IM, repeated after 4 hr if needed.
Contraindications
Patients who are opioid dependent; have recently received repeated doses of opioid analgesics; w/ head injury or elevated ICP; w/ significant resp depression or impaired resp function, including those receiving other CNS depressants or w/ underlying CNS disease affecting resp control; w/ acute MI, ventricular dysfunction, or coronary insufficiency when an increase in cardiac workload may pose significant risk; report of severe HTN during therapy.
Special Precautions
Caution in the elderly, those w/ hepatic or renal impairment, or during labor. Assess each patient's risk for opioid addiction, abuse, or misuse prior to prescribing. Closely monitor all patients for development of opioid addiction, abuse, or misuse throughout treatment. Use during pregnancy only if potential benefits to the mother outweigh possible risks to the baby. Weigh benefits of breastfeeding against the mother's need for the medication & any potential risks to the baby, either from the drug itself or condition of the mother, when deciding whether to use while breastfeeding. Closely monitor infants who are exposed through breast milk for signs of excessive drowsiness & difficulty breathing. W/drawal symptoms may occur in breastfed infants when the mother stops treatment or when breastfeeding is discontinued.
Adverse Reactions
Asthenia/lethargy, headache, sensation of heat; vasodilation, palpitations; anorexia, constipation, dry mouth, nausea &/or vomiting, stomach pain; anxiety, confusion, dizziness, euphoria, floating feeling, insomnia, nervousness, paresthesia, somnolence, tremor; cough, dyspnea; sweating, pruritus; blurred vision, ear pain, tinnitus, unpleasant taste.
Drug Interactions
Can lead to additive effects w/ benzodiazepines or other CNS depressants (eg, alcohol, sedatives, hypnotics, anxiolytics, tranquilizers, muscle relaxants, general anesth, antipsychotics, or other opioids). May increase risk of serotonin syndrome w/ medications that affect the serotonergic system eg, SSRIs, SNRIS, TCAs, triptans, 5-HT3 antagonists, serotonin-active agents (eg, mirtazapine, trazodone, tramadol), certain muscle relaxants (eg, cyclobenzaprine, metaxalone), & MAOIs including linezolid & IV methylene blue. Caution w/ CYP450 inhibitors or inducers eg, erythromycin or theophylline.
