Indications/Uses
Symptoms of idiopathic Parkinson's disease (paralysis agitans), postencephalitic parkinsonism, which may follow injury to nervous system by carbon monoxide & Mn intoxication.
Dosage/Direction for Use
Individualized dose. Tidomet 100 mg/25 mg tab Patient not previously treated w/ levodopa Initially 1 tab tid, increased gradually in increments of carbidopa 12.5 mg w/ levodopa 50 mg or carbidopa 25 mg w/ levodopa 100 mg daily or on alternate days as necessary. Maintenance: Carbidopa 75-200 mg w/ levodopa 750 mg to 2 g daily in divided doses. Max: Carbidopa 200 mg daily. Patient previously treated w/ levodopa 1/4 of dose previously being taken. Patient taking <1.5 g or >1.5 g levodopa Initially 1 tab tid or qid. Tidomet 250 mg/25 mg tab Initially 1 tab tid, may be increased by 1 tab/day or every other day as necessary, up to 8 tab/day. Tidomet CR tab Patient already receiving levodopa therapy & currently receiving levodopa alone Initially 1 tab bid adjusted according to response at intervals of not <3 days. Patient who is not already receiving initial dose Should not exceed 600 mg of levodopa. Patient already receiving conventional prep Initial dose similar to that of conventional prep but dosing intervals should be prolonged & normally between 4-12 hr. Alter dose & intervals according to response allowing at least 3 days between adjustments. Ave maintenance dose range: Carbidopa 100 mg w/ levodopa 400 mg to carbidopa 400 mg w/ levodopa 1.6 g.
Administration
Should be taken on an empty stomach: If GI distress occurs, take w/ food. CR Tab: Swallow whole, do not chew/crush.
Contraindications
Hypersensitivity. Narrow-angle glaucoma, patients w/ suspicious undiagnosed skin lesions or history of melanoma. Discontinue MAOIs at least 2 wk prior to initiating therapy.
Special Precautions
Discontinue use in patients receiving levodopa alone at least 8 hr before initiation of therapy. Patients w/ open-angle glaucoma, CV or pulmonary disease, endocrine disorders, psychiatric disturbances, osteomalacia, hepatic or renal disease, history of peptic ulceration, malignant melanoma. May impair ability to drive or operate machinery. Use in women of child bearing age after weighing possible hazard to the mother & child. Do not give to nursing mothers.
Adverse Reactions
GI effects eg, nausea, vomiting & anorexia. GI bleeding. Orthostatic hypotension. Psychiatric symptoms eg, agitation, anxiety, euphoria, nightmares, insomnia or sometimes drowsiness & depression. Aggression, paranoid delusions, hallucinations, delirium, severe depression, w/ or w/o suicidal behaviour, and unmasking of psychoses. Dyskinesias.
Drug Interactions
Symptomatic postural hypotension may occur w/ antihypertensive drugs. HTN & dyskinesia w/ tricyclics. May reduce therapeutic effects w/ phenothiazines & butyrophenones. +ve response to the direct Coombs' test may occur. Levodopa: Reversed beneficial effects w/ phenytoin & papaverine. May affect absorption w/ drugs that modify gastric emptying.
