Cefuroxime
Indications/Uses
Treatment of susceptible infections including UTI, resp tract, ENT, soft tissue, O & G infections, gonorrhoea. Prophylaxis against infection in abdominal, gynaecological, cardiac, pulmonary surgery where there is increased risk for infection.
Dosage/Direction for Use
IV/IM Adult Dose range: 1.5-6 g/day. Many infections will respond to 750 mg tid IM/IV. Should be increased to 1.5 g tid IV for more severe infections. Inj frequency can be increased to 6 hrly if necessary. Gonorrhoea 1.5 g as single dose. May be given as two 750-mg inj into different sites. Prophylaxis 1.5 g IV w/ induction of anaesth for abdominal & gynaecological operations, may be supplemented w/ two 750-mg IM doses 8 & 16 hr later. Cardiac & pulmonary operation 1.5 g IV w/ induction of anaesth continuing w/ 750 mg IM tid for a further 24-48 hr. Infant & childn 30-100 mg/kg/day given as 3 or 4 divided doses. Dose of 60 mg/kg/day will be appropriate for most infections. Patient w/ marked renal impairment (GFR 10-20 mL/min) 750 mg bid, severe renal impairment (GFR <10 mL/min) 750 mg once daily. Patient on dialysis A further 750 mg dose should be given at the end of each dialysis, continuous peritoneal dialysis 750 mg bid.
Contraindications
Hypersensitivity to cefuroxime or cephalosporins.
Special Precautions
Patients who have experienced allergic reaction to penicillins or other β-lactams. Increased risk of nephrotoxicity when combined w/ aminoglycosides; monitor renal function closely. Prolonged use may result in overgrowth of non-susceptible organisms. Carefully monitor patients w/ frequent loose stools for possible development of antibiotic colitis. Slight interference w/ Cu reduction methods (Benedict's, Fehling's, Clinitest). May cause false -ve reactions in ferricyanide test. May interfere w/ alkaline picrate assay for creatinine. Avoid concomitant use w/ furosemide; if co-administered, monitor renal function. Do not administer simultaneously w/ other medicines. Patients w/ renal impairment. Pregnancy & lactation.
Adverse Reactions
Rashes, thrombophlebitis after IV inj, GI disturbance & Candida intertrigo. Decreased Hb conc, eosinophilia, leucopenia, neutropenia. Increases in serum liver enzymes or serum bilirubin particularly in patients w/ pre-existing liver disease.
