Indications/Uses
Susceptible organisms: Staph aureus (including β-lactamase producing strains), Strep pneumoniae, Staph epidermidis, H. influenzae, Haemophilus parainfluenzae, E. coli, Proteus mirabilis, Pseudomonas aeruginosa, Proteus vulgaris, Proteus morganii, Neisseria gonorrhoeae, including both penicillinase- & nonpenicillinase-producing strains, Klebsiella pneumoniae. Strep pyogenes, Clostridium spp (excepting C. difficile), Serratia spp, Salmonella spp (including Salmonella typhi), Enterobacter spp, Shigella spp, Moraxella spp. Group B strep (Strep agalactiae), Viridans group streptococci, Peptostreptococcus, Haemophilus ducreyi, Neisseria meningitidis, Treponema pallidum, Fusobacterium, Bacteroides fragilis, Yersinia (including Yersinia enterocolitica), Providencia, Citrobacter, Alcaligenes, Aeromonas, Plesiomonas shigelloides. Lower resp tract infection; acute bacterial otitis media; skin & skin structure infections; UTI (complicated & uncomplicated); uncomplicated gonorrhea (cervical/urethral & rectal); pelvic inflammatory disease caused by Neisseria gonorrheae; bone & joint infections; intra-abdominal infections; meningitis, septicemia.
Dosage/Direction for Use
IM/IV Adult 1-2 g once daily (or in equally divided doses bid). Should not exceed 4 g daily. Meningitis Initially 100 mg/kg. Recommended total dose: 100 mg/kg/day once daily (or in equally divided doses every 12 hr). Not to exceed 4 g. Uncomplicated gonococcal infection 250 mg IM as a single dose. Pre-op (surgical prophylaxis) 1-2 g IV as a single dose 2 hr before surgery. Infant & childn <12 yr 20-80 mg/kg/day once daily. Administer doses >50 mg/kg by IV infusion over 30 min. Neonate <14 days 20-50 mg/kg/day once daily. Should not exceed 50 mg/kg/day. Patient w/ estimated CrCl <10 mL/min Should not exceed 2 g daily.
Contraindications
Hypersensitivity to ceftriaxone; cephalosporin or penicillin class of antibiotics; adrenalin class of local anesth eg, lidocaine (in IM administration only).
Special Precautions
Discontinue in patients who develop signs & symptoms suggestive of gallbladder disease &/or sonographic abnormalities. Caution in patients who has demonstrated some form of allergy to cephalosporins, penicillins or other drugs. +ve Coombs' test. False +ve reaction to glucose using reducing substances. False +ve galactosemia test. Periodically monitor serum drug conc in patients w/ renal failure & hepatic dysfunction.
Adverse Reactions
Rash; eosinophilia, thrombocytosis & leukopenia; elevations of SGOT or SGPT; elevations of BUN; diarrhea. Shock; pruritus, fever or chills; anemia, hemolytic anemia, neutropenia, lymphopenia, thrombocytopenia & prolongation of prothrombin time; elevations of alkaline phosphatase & bilirubin; elevations of creatinine & presence of casts in urine; nausea or vomiting, & dysgeusia.
Drug Interactions
Cephalosporin antibiotics at high dosage combined w/ aminoglycoside antibiotics or potent diuretics eg, furosemide may have an adverse effect on renal function.
