Amikacin sulfate
Indications/Uses
Bacterial septicemia including neonatal sepsis. Serious infections of the resp tract. Infections of the bone & joints. Intra-abdominal infections including peritonitis. Burns & post-op infections. Serious & complicated UTI due to susceptible organisms. As initial therapy in suspected gm -ve infections before the results of susceptibility testing are obtained.
Dosage/Direction for Use
Adult, childn & older infant 15 mg/kg/day given as a single dose or divided into 2-3 equal doses administered at equally divided intervals. Uncomplicated UTI Single dose of 500 mg daily or 250 mg bid. Newborn Loading dose: 10 mg/kg, followed by 7.5 mg/kg every 12 hr; 0-7 days, <2 kg 7.5 mg/kg every 18-24 hr for 7-10 days. Peritoneal dialysis 7.5 mg/kg. Renal impairment CrCl >80 mL/min 15 mg/kg every 24 hr, 60-80 mL/min 12 mg/kg every 24 hr, 40-60 mL/min 7.5 mg/kg every 24 hr, 30-40 mL/min 4 mg/kg every 24 hr, 20-30 mL/min 7.5 mg/kg every 48 hr, 10-20 mL/min 4 mg/kg every 48 hr, <10 mL/min 3 mg/kg every 72 hr.
Contraindications
Hypersensitivity to aminoglycosides.
Special Precautions
Avoid concurrent use w/ other ototoxic or nephrotoxic agents. Observe for signs of 8th cranial nerve damage during therapy. Assess kidney function prior to starting therapy & daily during the course of treatment. Increase hydration if signs of renal irritation appear. Stop treatment if azotemia increases or if progressive decrease in urinary output occurs. Patients w/ neuromuscular disorders eg, myasthenia gravis or parkinsonism. Overgrowth of nonsusceptible organisms. Contains metabisulfite. Pregnancy & lactation. Premature neonates. Elderly.
Adverse Reactions
Auditory, vestibular, renal toxicity & neuromuscular blockade. Hearing loss, loss of balance; acute muscular paralysis, apnea; elevated serum creatinine, albuminuria, presence of casts, RBC & WBC in urine, azotemia, oliguria.
Drug Interactions
Avoid concurrent &/or sequential use w/ other drugs w/ similar toxic potential (eg, other aminoglycosides, acyclovir, amphotericin B, bacitracin, capreomycin, cephalosporins, colistin, cisplatin, methoxyflurane, polymyxin B, vancomycin). Increased risk of ototoxicity w/ ethacrynic acid, furosemide, urea, mannitol. May potentiate neuromuscular blockade & cause resp paralysis w/ general anesth of neuromuscular blocking agents (eg, succinylcholine, tubocurarine). May potentiate effects of oral anticoagulants w/ oral neomycin. Additive or synergistic effect w/ β-lactam antibiotics or vancomycin & extended-spectrum penicillins.
