Atorvastatin.
Description
Each film-coated tablet contains 10 mg atorvastatin.
Each film-coated caplet contains 20 mg and 40 mg atorvastatin.
Indications/Uses
Adjunct therapy along with a diet to reduce total cholesterol, LDL-C, apo B, and triglycerides in patients with primary hypercholesterolemia, mixed hyperlipidemia, and heterozygous and homozygous familial hypercholesterolemia, when the response to diet and other non-pharmacological management is inadequate. Prevention of cardiovascular complications in hypertensive patients aged ≥40 with dyslipidemia and at least three risk factors such as LVH, ECG abnormalities, NIDDM, peripheral vascular disease, recent cerebrovascular disease including TIA, microalbuminuria/proteinuria, regular smoking, TC/HDL-C ratio ≥6, or a family history of CAD in a first-degree relative before age 55 (male) or 60 (female). Adjunct to diet to reduce TC, LDL-C, and apo B in boys and postmenarchal girls aged 10–17 with heterozygous familial hypercholesterolemia, if LDL-C remains ≥190 mg/dL, or ≥160 mg/dL with a family history of premature cardiovascular disease or two or more cardiovascular risk factors after adequate dietary intervention.
Dosage/Direction for Use
Initially 10 mg once daily. Dosage range: 10-80 mg once daily.
Primary hypercholesterolemia and mixed hyperlipidemia: 10 mg daily.
Homozygous familial hypercholesterolemia: 80 mg.
Heterozygous familial hypercholesterolemia: Children 10-17 years: 10 mg daily, maximum dose: 20 mg daily. Adjusment dose is performed at ≥4 weeks interval. Concomitant use w/ cyclosporine ≤10 mg.
Contraindications
Hypersensitivity. Active liver disease or persistent elevations in serum transaminases >3 x ULN.
Special Precautions
Perform liver function tests before starting, 12 weeks after, during dose evaluation, and regularly thereafter. Patients with diffuse myalgia, muscle pain or weakness, and/or significant increases in CPK are advised to promptly report muscle pain, tenderness, or weakness, especially if accompanied by malaise or fever. Atorvastatin therapy should be discontinued if there is a significant increase in CPK levels or if myopathy is suspected or diagnosed, or in the presence of risk factors for developing secondary renal failure due to rhabdomyolysis.
Adverse Reactions
Insomnia, headache, nausea, diarrhea, abdominal pain, dyspepsia, constipation, flatulence, myalgia, arthralgia, asthenia.
Drug Interactions
Concomitant use with cytochrome P450 3A4 inhibitors, transporter inhibitors (cyclosporine), erythromycin/clarithromycin, protease inhibitors, diltiazem hydrochloride, itraconazole, cytochrome P450 3A4 inducers, antacids, colestipol, digoxin, azythromycin, oral contraceptives containing norethindrone and ethinyl estradiol, colchicine, amlodipine, fusidic acid.
