Indications/Uses
Adjunct to diet for the reduction of total cholesterol (TC), LDL-C, apo B & triglycerides in patients w/ primary hypercholesterolemia, mixed hyperlipidemia, heterozygous familial hypercholesterolemia & HoFH when response to diet & other non-pharmacological management is inadequate. Prevention of CV complication in hypertensive patients ≥40 yr w/ dyslipidemia & at least 3 risk factors eg, LVH, ECG abnormalities, NIDDM, peripheral vascular disease, recent cerebrovascular disease including TIA, microalbuminuria/proteinuria, regular smoking, TC/HDL-C ratio ≥6, or a family history of CAD in a 1st-degree relative before 55 yr (male) or 60 yr (female). Adjunct to diet for the reduction of TC, LDL-C, & apo-B in boys & postmenarchal girls 10-17 yr w/ heterozygous familiar hypercholesterolemia, if LDL-C remains ≥190 mg/dL, or ≥160 mg/dL w/ family history of premature CV disease or ≥2 risk factors after adequate dietary intervention.
Dosage/Direction for Use
Initially 10 mg once daily. Dosage range: 10-80 mg once daily. Primary hypercholesterolemia & mixed hyperlipidemia 10 mg daily. HoFH 80 mg daily. Heterozygous familial hypercholesterolemia Childn 10-17 yr 10 mg daily. Max dose: 20 mg daily. Dose adjustment is made at intervals of ≥4 wk.
Administration
May be taken with or without food: Avoid excessive consumption (>1 L/day) of grapefruit juice.
Contraindications
Hypersensitivity. Active liver disease or persistent elevations of serum transaminases >3x ULN.
Special Precautions
Discontinue therapy if there is a significant increase in CPK levels or if myopathy is suspected or diagnosed; in the presence of risk factor of secondary renal failure due to rhabdomylosis. Perform LFT before starting, 12 wk after, during dose evaluation, & regularly thereafter. Patients w/ diffuse myalgia, muscle pain or weakness, &/or significant increase in CPK; promptly report muscle pain, tenderness, or weakness especially if accompanied by malaise or fever.
Adverse Reactions
Insomnia, headache, nausea, diarrhea, abdominal pain, dyspepsia, constipation, flatulence, myalgia, arthralgia, asthenia.
Drug Interactions
Concomitant use w/ CYP3A4 inhibitors, transporter inhibitors (cyclosporine), erythromycin/clarithromycin, PIs, diltiazem HCl, itraconazole, CYP3A4 inducers, antacids, colestipol, digoxin, azithromycin, OC containing norethindrone & ethinyl estradiol, colchicine, amlodipine, fusidic acid.
