Paracetamol 500 mg, phenylephrine HCl 10 mg, chlorpheniramine maleate 2 mg
Indications/Uses
Symptomatic relief of nasal congestion, sneezing, runny nose, fever & headache due to colds, sinusitis, hay fever or other upper resp tract allergies.
Dosage/Direction for Use
Adult 1 tab every 4 hr. Childn 6-12 yr ½ tab every 4 hr.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to paracetamol, phenylephrine HCl, & chlorpheniramine maleate. Patients receiving MAOIs.
Special Precautions
Discontinue use if swelling of face, eyelid, lips, urticaria, skin (usually erythematous) & maculopapular rash, & TEN have developed. Not to be used >5 days. May cause drowsiness, dizziness & psychomotor impairment; hepatotoxicity in overdosage. Mouth dryness, urinary retention, thickening of bronchial secretions, blurred vision, nervousness & blood dyscrasia; sedation, delirium, hypotension, restlessness, insomnia, paradoxical reaction characterized by even seizures. Alcoholic patients. Patients w/ HTN, CV disease, hyperthyroidism, experience of asthma & bronchiectasis; chronic alcoholism, diabetes, heart or thyroid disease, high BP, chronic lung disease, glaucoma, difficulty in urination due to enlarged prostate. Not to be used concomitantly w/ alcohol or alcohol containing beverages. Avoid use w/ any other drug containing paracetamol. Concomitant use w/ antidepressants, other antihistamines, tranquilizers, or sedatives; hypnotics, psychotherapeutics or other drugs w/ CNS depressant effects (eg, benzodiazepines); anticholinergic drugs. Not to drive, use machinery & perform any hazardous activities during drug administration. Serious liver or kidney disease. 1st trimester of pregnancy; use during 1st 2 trimesters only when clearly needed. Lactation. Childn <1 yr. Elderly.
Adverse Reactions
Drowsiness, restlessness, dry mouth, nervousness, visual disturbances, dermatitis, weakness & nausea; nephropathy, including papillary renal failure following consumption of large amount of acetaminophen; renal tubular necrosis occasionally associated w/ hepatic injury produced by acetaminophen overdose; neutropenia & thrombocytopenia purpura; hepatic toxicity w/ acetaminophen overdose; increased risk of hepatotoxicity in alcoholic & malnourished patients, patients w/ liver disease & patients taking drugs that induce hepatic microsomal enzymes. Aggravated bronchospasm in patients sensitive to ASA or other analgesics.
Drug Interactions
Paracetamol: Increased hepatotoxicity & reduced therapeutic effects w/ barbiturates (eg, phenobarb). Increased risk of paracetamol-induced liver damage w/ ethanol. Chlorpheniramine maleate: Potentiated effects w/ alcohol, CNS depressants, TCAs. Prolonged & intensified antimuscarinic effects w/ MAOIs. May mask symptoms of ototoxicity w/ ototoxic medications, particularly ototoxic antibiotics. Increased drowsiness effect w/ alcohol, sedatives & tranquilizers. Phenylephrine HCl: Not to be used w/ MAOIs or for 2 wk after stopping MAOI.
