Indications/Uses

Acute evolving transmural MI, DVT, pulmonary embolism, arterial thrombosis or embolism & occlusion of arterio-venous cannulae.

Dosage/Direction for Use

Acute evolving transmural MI 1.5 M IU IV infusion over 30-60 min alternatively 10,000-30,000 IU intracoronary bolus over 15 sec to 2 min maintained by 2,000-4,000 IU/min for 60 min. Acute DVT, pulmonary embolism, arterial thrombosis or embolism Initially 250,000 IU over 30 min followed by maintenance dose of 0.1 M IU/hr for 24-72 hr. Occlusion of arteriovenous cannulae 250,000 IU direct infusion into the cannula over 25-35 min.

Contraindications

Invasive or traumatic procedures w/in the previous 10 days, HTN, diabetic retinopathy, cerebral apoplexy w/in 2 mth, transient ischaemic attacks, history of peptic ulceration, ulcerative colitis, diverticulitis, GI lesion, thrombocytopenia, severe hepatic or renal impairment, active infective endocarditis, atrial valve disease w/ atrial fibrillation, previous streptokinase therapy >5 days & up to 6 mth, previous or recent Streptococcal infection, active TB & deformity, coma.

Special Precautions

Avoid IM inj for the 1st 24 hr. May cause hypotension w/ rapid rate of infusion. Administer anticoagulants 4 hr after discontinuation of treatment to prevent re-thrombosis. Pregnancy & lactation. Elderly.

Adverse Reactions

Hypotension, allergic reaction, bleeding, excessive haemorrhage; allergic reaction, slight to moderate elevation in body temp, hypotensive, bradycardia.

Drug Interactions

May cause bleeding complications w/ heparin. Increased risk of bleeding w/ drugs affecting blood platelet function eg, salicylic acid prep, pyrazolone or indole derivatives.
