Indications/Uses
Monotherapy in partial seizures w/ & w/o secondary generalization in adults & childn ≥12 yr. Adjunctive therapy in partial seizures w/ & w/o secondary generalization in adults & childn ≥3 yr. Neuropathic pain in adults ≥18 yr.
Dosage/Direction for Use
Epilepsy Adult & ped patient >12 yr 900-3,600 mg daily. Initially 300 mg tid on Day 1, or titrate dose to 300 mg once daily on Day 1, 300 mg bid on Day 2 & 300 mg tid on Day 3. May be increased to 3 equally divided doses. Max: 3,600-4,800 mg daily. Max time interval: ≤12 hr. Ped patient 3-12 yr Initially 10-15 mg/kg daily in 3 equally divided doses for approx 3 days, ≥5 yr 25-35 mg/kg daily in 3 equally divided doses, 3 to <5 yr 40 mg/kg daily in 3 equally divided doses. Max: 50 mg/kg daily. Max time interval: ≤12 hr. Neuropathic pain Adult Initially 900 mg daily in 3 equally divided doses. Dose may be increased if necessary. May initiate therapy by titrating dose to 300 mg once daily on Day 1, 300 mg bid on Day 2, & 300 mg tid on Day 3. Max: 3,600 mg daily. Renal impairment w/ CrCl ≥80 mL/min 900-3,600 mg daily tid, 50-79 mL/min 600-1,800 mg daily tid, 30-49 mL/min 300-900 mg daily tid, 15-29 mL/min 150 mg (administered as 300 mg every other day) to 600 mg daily, <15 mL/min 150 mg (administered as 300 mg every other day) to 300 mg daily. Patient undergoing hemodialysis Loading dose: 300-400 mg, then 200-300 mg following each 4 hr of hemodialysis.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Discontinue treatment if signs or symptoms of anaphylaxis (eg, difficulty of breathing, swelling of lips, throat & tongue, & hypotension) occur. Evaluate patients immediately if signs or symptoms of hypersensitivity (eg, fever or lymphadenopathy) are present, & discontinue if alternative etiology cannot be established. Not for treatment of absence seizures. Abrupt w/drawal may precipitate status epilepticus. Increased occurrence of accidental injury (fall); confusion, loss of consciousness & mental impairment. DRESS. W/drawal symptoms including anxiety, insomnia, nausea, pains, sweating, tremor, headache, depression, feeling abnormal, dizziness & malaise. May cause hematologic disorder. Patients w/ current or history of substance abuse. Evaluate patients for history of drug abuse &/or psychiatric disorder. Monitor patients for signs & symptoms of abuse or dependence eg, development of tolerance, dose escalation & drug-seeking behavior. Observe for signs of CNS depression eg, somnolence, sedation & resp depression in patients requiring concomitant treatment w/ CNS depressants including opioids. May affect ability to drive or operate machines. Liver & kidney disease. Women of childbearing potential should use effective contraception during treatment. Not to be used during pregnancy. May cause major birth defects during 1st trimester of pregnancy. Lactation. May increase risk of neonatal w/drawal syndrome in newborns exposed in utero & co-exposure to opioids. Ped patients <3 yr for adjunctive therapy in epilepsy; childn <12 yr for monotherapy in epilepsy. Patients <18 yr for neuropathic pain.
Adverse Reactions
Abdominal & back pain, fatigue, fever, headache, viral infection, asthenia, malaise, facial edema, accidental injury, flu syndrome, infection, pain; vasodilation, HTN; constipation, dental abnormalities, diarrhea, dyspepsia, increased appetite, dry mouth or throat, nausea &/or vomiting, flatulence, anorexia, gingivitis; leukopenia, decreased WBC, purpura; peripheral edema, increased wt, wt gain; fracture, myalgia, arthralgia; amnesia, ataxia, confusion, abnormal coordination, thinking & gait, depression, dizziness, dysarthria, emotional lability, insomnia, nervousness, nystagmus, somnolence, tremor, twitching, vertigo, hyperkinesia, increased, decreased or absent reflexes, paresthesia, anxiety, hostility, hypesthesia; coughing, pharyngitis, rhinitis, pneumonia, bronchitis, resp infection, dyspnea; abrasion, acne, pruritus, rash; amblyopia, diplopia, abnormal vision; impotence, UTI.
Drug Interactions
Resp depression &/or sedation w/ CNS depressants including opioids. Reduced bioavailability w/ Al- & Mg-containing antacids. False +ve results w/ Ames N-Multistix SG dipstick test w/ other anticonvulsant drugs.
