Enalapril maleate.
Description
Each tablet contains Enalapril maleate 5 mg and 20 mg.
Action
ENARIL is an angiotensin converting enzyme (ACE) inhibitor. The effect of a dose will gradually take place within one hour and will usually stay for 24 hours, daily dosage may given in one or two single administrations. Enalapril is a prodrug and will be transformed to enalaprilat which is active by liver. ENARIL can reduce blood pressure in normal and hypertensive person by reducing peripheral vascular resistant but has no effect to heart rate. For the patient without congestive heart failure, ENARIL has no effect or little effect to cardiac output and does not interfere renal blood flow or glomerular filtration rate.
Indications/Uses
High blood pressure, whether alone or in combination with other anti-hypertensive drugs especially diuretics.
Congestive heart failure.
Dosage/Direction for Use
Usual dosage is 10-40 mg daily, by once or twice daily administrations but not more than 40 mg daily, or as directed by the physicians.
For patients with diuretic pre-treatment, the diuretics should be suspended for 2-3 days before the beginning of ENARIL therapy. If blood pressure is not controlled, the diuretics can be given.
Special Precautions
ENARIL should be used with caution for the simultaneous intake with potassium supplement or potassium sparing diuretics, all drugs which may increase the resistance of blood vessels.
For the patients with renal dysfunction, the dosage must be reduced.
For the patients with liver dysfunction, the active metabolite of enalapril becomes low and the dosage may be increased but there is no sufficient experience in these patients.
ENARIL should not be recommended for patients who show over-sensitivity reaction under ENARIL therapy.
Use in Pregnancy & Lactation: ENARIL should be used with caution in pregnancy and lactating woman.
Use in Children: ENARIL should not be recommended for children because there is no sufficient report of safety of the drug in children.

Adverse Reactions
ENARIL is generally well tolerated but may cause: Hypotensive especially with high dosage or simultaneous intake with diuretics or dehydration states or salt-limited diet.
Renal function disturbance such as glycosuria or proteinuria and in some cases may lead to acute renal failure.
Leukopenia, but increased eosinophils.
Hyperkalemia.
