Silver sulfadiazine
Indications/Uses
Adjunct for the prevention & treatment of wound sepsis in patients w/ 2nd- & 3rd-degree burns.
Dosage/Direction for Use
Apply once daily to bid to approx 1/16 inch thickness. Reapply immediately after hydrotherapy. Continue until satisfactory healing has occurred or until burn site is ready for grafting.
Contraindications
Hypersensitivity to silver sulfadiazine. Should not be used on pregnant women approaching or at term due to the possibility of kernicterus, on premature or newborn infants during the first 2 mth of life.
Special Precautions
Blood dyscracias, including agranulocytosis, aplastic anemia, thrombocytopenia, leukopenia & hemolytic anemia; dermatologic & allergic reactions including SJS, TEN & exfoliative dermatitis; GI reactions; hepatitis & hepatocellular necrosis; CNS reactions & toxic nephrosis. Potential cross-sensitivity between silver sulfadiazines & other sulfonamides. Fungal proliferation in & below eschar may occur. May be hazardous to individuals w/ G6PD individuals due to hemolysis. Discontinue if hepatic & renal functions become impaired & elimination of drug decreases. Carefully monitor renal functions & check for sulfa crystals in urine. May affect interpretation of lab test affecting serum osmolality w/ propylene glycol vehicle. Discontinue nursing or drug due to possibility of kernicterus.
Adverse Reactions
Transient leukopenia. Skin necrosis/discoloration, erythema multiforme, burning sensation, rashes & interstitial nephritis.
Drug Interactions
May potentiate the effects of oral anticoagulants, MTX & phenytoin. Possibility of interactions w/ other highly protein-bound drugs eg, NSAIDs. Enhanced effects of sulfonylurea. Decrease plasma-ciclosporin conc when used concomitantly. Failures of hormonal contraceptives resulting to pregnancy. May increase risk of crystalluria w/ compounds which render urine acidic.
