Cefdinir
Indications/Uses
Gonorrhea, otitis media, pharyngitis, lower resp tract infections eg, bronchitis, & UTI due to susceptible microorganisms. Powd for susp: Acute maxillary sinusitis in ped patients (6 mth-12 yr).
Dosage/Direction for Use
Adult 600 mg daily as single dose or in 2 divided doses. Childn 14 mg/kg daily up to max of 600 mg daily. Renal impairment Reduced doses may be needed.
Administration
May be taken with or without food.
Contraindications
Patients w/ known allergy to cephalosporins.
Special Precautions
Reddish stools in patients given w/ Fe supplements. Use during pregnancy only if clearly needed. Powd for susp: Determine whether patient has had previous hypersensitivity reactions to other cephalosporins, penicillins, or other drugs before therapy is instituted. Exercise caution if to be given to penicillin-sensitive patients because cross hypersensitivity among β-lactam antibiotics may occur. Discontinue if allergic reaction occurs. Consider Clostridium difficile associated diarrhea (CDAD) in all patients who present w/ diarrhea following antibiotic use. Discontinue ongoing antibiotic use not directed against C. difficile if CDAD is suspected or confirmed. Institute appropriate fluid & electrolyte management, protein supplementation, antibiotic treatment, & surgical evaluation as clinically indicated. Superinfection. Use w/ caution if patient suffers from seizures; stop medication if patient has seizure while using cefdinir. Labor & delivery. Safety & efficacy have not been established in neonates & infants <6 mth.
Adverse Reactions
Cap: Hypersensitivity reactions, including skin rashes, urticaria, eosinophilia, fever, reactions resembling serum sickness & anaphylaxis. +ve response to Coombs test. Neutropenia & thrombocytopenia. Powd for susp: Diarrhea, nausea, vag infection & rash. Colitis.
Drug Interactions
Decreased absorption w/ antacids or Fe supplements. Reduced renal excretion w/ probenecid.
