Phenoxymethylpenicillin K
Indications/Uses
Resp tract, GUT, skin & skin structure infections, oral & dental infections including necrotizing gingivitis caused by susceptible organisms. Prevention & treatment of secondary bacterial infections in measles, mumps, chicken pox, flu & other viral infections. Prophylaxis in recurrent streptococcal infections including prevention of recurrence following rheumatic fever &/or Syndenham's chorea, & to prevent bacterial endocarditis after tonsillectomy or tooth extraction in patients w/ history of rheumatic fever &/or congenital heart disease.
Dosage/Direction for Use
Duration of treatment: 10-14 days. Adult & childn ≥12 yr Mild to moderately severe infections 250-500 mg every 4-6 hr. Prevention of recurrence following rheumatic fever 250 mg every 12 hr continuously. Childn 6-12 yr (who can swallow whole cap) 250 mg every 6 hr.
Administration
Should be taken on an empty stomach: Take on an empty stomach 1 hr before or 2 hr after meals.
Contraindications
Hypersensitivity to penicillins &/or cephalosporins. Chronic, severe or deep-seated infections.
Special Precautions
Not to be used as adjunctive prophylaxis for genitourinary instrumentation or surgery, lower intestinal tract surgery, sigmoidoscopy or complications of childbirth. Previous hypersensitivity to penicillins, cephalosporins or other allergens. Discontinue use if allergic reaction occurs. History of allergy &/or asthma. C. difficile-associated diarrhea & colitis; patients w/ diarrhea prior initiation of therapy. Patients w/ severe infections or w/ nausea, vomiting, gastric dilatation, esophageal achalasia, cardiospasm or intestinal hypermotility. Use of an alternative or additional method of contraception if concomitantly use w/ estrogen-containing OC. Long term or repeated use; overgrowth of non-susceptible organisms including fungi. May interfere w/ urinary tests for fructose, aminolevulinic acid, estrogens, 17-hydroxycorticosteroids, protein & sugar, w/ serum tests for uric acid; w/ biological tests for serum folate; & w/ serum electrophoresis. May decrease urinary excretion of aminohippurate Na & phenolsulfonphthalein by competing for renal tubular secretion w/ these diagnostic agents. Renal impairment. Pregnancy & lactation.
Adverse Reactions
Anaphylaxis (manifested by generalized pruritus, angioneurotic edema which may affect the larynx, tachycardia, severe dyspnea, cyanosis, diaphoresis, stridor, dizziness, rigors, loss of consciousness & peripheral circulatory failure), acute interstitial nephritis, agranulocytosis, allergic bronchial asthma, allergic vasculitis, angioedema, arthus phenomenon, chills, drug fever, eosinophilia, erythema multiforme, erythema nodosum, exfoliative dermatitis, fixed drug eruptions, hemolytic anemia, hypotension, laryngospasm, leukopenia, urticarial, erythematosus or morbiliform (maculopapular or exanthematic) rash & pruritus, serum sickness-like reactions (characterized by fever, malaise, urticaria, arthralgia, myalgia, lymphadenopathy, splenomegaly), SJS, vesiculobullous eruptions. Transient neutropenia, thrombocytopenia, thrombocytopenic purpura. Black hairy tongue, C. difficile-associated diarrhea & colitis, increased capillary fragility characterized by spontaneous petechial hemorrhages w/ +ve result on the tourniquet test, diarrhea, epigastric distress, nausea, pruritus ani, sore mouth or tongue, vomiting. Hepatotoxicity, nephropathy, neuropathy.
Drug Interactions
Additive or synergistic antibacterial activity w/ aminoglycosides. May occasionally be additive or partially synergistic & antagonism may also occur w/ other β-lactams (eg, cephalosporins, 1-oxa-β-lactams, cephamycins); bacteriostatic antibiotics (eg, erythromycin, chloramphenicol, tetracyclines). May decrease efficacy of estrogen-containing OCs & increase incidence of breakthrough bleeding. Reduction in tubular excretion leading to increased plasma conc w/ probenecid. May be displaced from binding sites by salicylates & other NSAIDs, & vice versa. Inhibited serum-binding w/ sulfamethoxypyridazine, sulfaethidole. Reduced absorption w/ antacids. Generally delaying absorption w/ food. Reduced activity against Staph aureus in the presence of Zn oxide & to a lesser extent w/ Mg carbonate, Mg oxide or calamine.
