Duloxetine HCl
Indications/Uses
Major depressive disorder, generalized anxiety disorder, fibromyalgia & relief of diabetic peripheral neuropathic pain in adults.
Dosage/Direction for Use
Major depressive disorder 30-60 mg daily. Can be increased by 30 mg/day. Max: 120 mg/day. Generalized anxiety disorder, fibromyalgia, diabetic peripheral neuropathy pain 30 mg cap once daily for 1 wk then 60 mg once daily. Can be increased by 30 mg/day. Max: 120 mg/day.
Administration
May be taken with or without food: Swallow whole, do not chew/crush/open cap.
Contraindications
Hypersensitivity. Concomitant use w/ MAOIs, serotonin precursors eg, tryptophan & thioridazine. Hepatic insufficiency, end-stage renal disease, severe renal impairment, uncontrolled narrow-angle glaucoma. Lactation.
Special Precautions
History of mania, seizures, narrow angle glaucoma & liver dysfunction. Patients prone to suicide. Gradual reduction in dose is recommended. At least 14 days should elapse between discontinuation of MAOI & 5 days before initiating MAOI. Closely monitor patients for clinical worsening, suicidality & unusual changes in behaviour. Discontinue if jaundice or other evidence of clinically significant liver dysfunction, symptomatic orthostatic hypotension &/or syncope, symptomatic hyponatremia or urinary hesitancy & retention develop. Measure BP prior to therapy & monitor throughout treatment period. Discontinue any serotonergic or antidopaminergic agents. Use w/ aspirin, NSAIDs, warfarin & other anti-coagulants. Patients w/ substantial alcohol use. Smoking. Pregnancy. Childn.
Adverse Reactions
Nausea, vomiting, dizziness, somnolence, headache, dry mouth, constipation, diarrhea, decreased appetite, hyperhidrosis, fatigue. Palpitations; vertigo; vision blurred; flatulence, dyspepsia, loose stools; chills/rigors, anorexia, appetite & decreased wt; increased wt; musculoskeletal pain/spasms, muscle cramp, myalgia; dysgeusia, lethargy, tremor, migraine & parasthesia/hypoesthesia; abnormal dreams, decreased libido & sleep disorder; anorgasmia/orgasm abnormal, erectile dysfunction, ejaculation delayed or ejaculation disorder, penis disorder; cough, pharyngolaryngeal pain & yawning; hot flush. Completed suicide.
Drug Interactions
TCAs, phenothiazine, type 1C antiarrhythmics & other CNS agents.
