Loperamide hydrochloride.
Each capsule contains Loperamide Hydrochloride 2 mg.
Loperamide hydrochloride inhibits intestinal motility and is used in the management of acute and chronic diarrhea. Adverse effects include toxic megacolon, dry mouth, dizziness, fatigue and hypersensitivity reactions; CNS depression may occur in overdosage.
Pharmacology: Pharmacokinetics: Absorption and Fate: About 40% of a dose of loperamide is reported to be absorbed from the gastrointestinal tract to undergo first-pass metabolism in the liver and excretion in the faeces via the bile as inactive conjugate; there is slight urinary excretion. The elimination half-life is reported to be about 10 hours.
Loperamide inhibits peristalsis and gastrointestinal secretions and is used in the management of acute and chronic diarrhea as well as in illeostomy management to control the volume of discharge. However, the main aim in treating acute diarrhea is the correction of fluid and electrolyte depletion with rehydration therapy this is especially important in infants and young children.
In acute diarrhea the usual initial dose for adults is 4 mg followed by 2 mg after each loose stool; the usual daily dose is 6 to 8 mg and 16 mg daily should not be exceeded.
Loperamide should be used with caution in patients with hepatic dysfunction because of its considerable first-pass metabolism in the liver. It should also be used with caution in young children because of a greater variability of response in this age group. And it is not generally recommended for use in infants.
Patients with inflammatory bowel diseases or pseudomembranous colitis receiving loperamide should be carefully observed for signs of toxic megacolon. It should not be used in patients with dysentery or if constipation must be avoided.
Abdominal pain and other gastrointestinal disturbances, including toxic megacolon, dry mouth dizziness, fatigue. and hypersensitivity reactions including skin rashes have been reported. Depression of the CNS. to which children may be more sensitive, may be seen in overdosage, and naloxone hydrochloride has been recommended for its treatment. Gastric lavage and activated charcoal are also used in the management of loperamide overdosage.
