Loperamide should be used with caution in patients with hepatic dysfunction because of its considerable first-pass metabolism in the liver. It should also be used with caution in young children because of a greater variability of response in this age group. And it is not generally recommended for use in infants.
Patients with inflammatory bowel diseases or pseudomembranous colitis receiving loperamide should be carefully observed for signs of toxic megacolon. It should not be used in patients with dysentery or if constipation must be avoided.
