Adult Severe or life-threatening hemorrhage (eg, during anticoagulant therapy) 5-10 mg slow IV given at least 30 sec together w/ fresh frozen plasma (FFP) or prothrombin complex conc (PCC). May be repeated as needed.
Patient w/ asymptomatic high INR w/ or w/o mild hemorrhage INR 5-9: 0.5-1 mg + warfarin, INR >9: 1 mg + warfarin; INR 5-8 & >8: 1-2 mg + acenocoumarol; INR 5-9 & >9: 2-5 mg + phenprocoumon, INR >10 Individually adapted doses.
Elderly 0.5-22 mg IV.
Major bleeding 5-10 mg + warfarin + FFP or PCC; 5 mg + acenocoumarol + FFP or PCC; 5 mg or 10 mg + phenprocoumon + PCC.
Life-threatening bleeding 10 mg + warfarin + FFP, PCC or recombinant factor VIIa.
