Indications/Uses
Symptomatic control of acute & chronic diarrhea. Reduce number & vol of stools & harden its consistency in patients w/ ileostomy.
Dosage/Direction for Use
Acute diarrhea Adult Initially 2 cap (4 mg) followed by 1 cap (2 mg) after every subsequent loose stool. Max: 8 cap (16 mg) daily. Childn Initially 1 cap (2 mg) followed by 1 cap (2 mg) after every subsequent loose stool. Max: 3 cap/20 kg but should not exceed 8 cap daily. Chronic diarrhea Adult Initially 2 cap (4 mg) daily, adjusted until 1-2 solid stools daily are obtained. Maintenance: 1-6 cap (2-12 mg) daily. Max: 8 cap (16 mg) daily. Childn Initially 1 cap (2 mg) daily, adjusted until 1-2 solid stools daily are obtained. Maintenance: 1-6 cap (2-12 mg) daily. Max: 3 cap/20 kg but should not exceed 8 cap daily.
Administration
May be taken with or without food: Take w/ liqd.
Contraindications
Hypersensitivity. Not to be used as primary therapy in patients w/ acute dysentery; acute ulcerative colitis; bacterial enterocolitis caused by invasive organisms including Salmonella, Shigella & Campylobacter; pseudomembranous colitis associated w/ the use of broad-spectrum antibiotics. Not be used when inhibition of peristalsis is to be avoided due to possible risk of significant sequelae including ileus, megacolon & toxic megacolon. Promptly discontinue when constipation, abdominal distension or ileus develops. Childn <2 yr.
Special Precautions
Discontinue administration in acute diarrhea if clinical improvement is not observed w/in 48 hr. Stop therapy at the earliest signs of abdominal distension in AIDS patients. Abuse & misuse have been described in individuals w/ opioid addiction. Use caution when driving a car or operating machinery. Use w/ caution in patients w/ hepatic impairment as it may result in relative overdose leading to CNS toxicity. Not advisable to administer in pregnancy. Advise women who are pregnant or breast-feeding to consult for appropriate treatment. Not to be given to childn 2-6 yr w/o medical prescription & supervision.
Adverse Reactions
Headache, constipation, flatulence, nausea, dizziness, dry mouth, abdominal pain/discomfort. Acute diarrhea: Upper abdominal pain, abdominal distension, rash, vomiting & somnolence. Chronic diarrhea: Dyspepsia.
Drug Interactions
Increased plasma levels w/ quinidine or ritonavir (P-gp inhibitors); itraconazole or ketoconazole (CYP3A4 & P-gp inhibitors); gemfibrozil (CYP2C8 inhibitor). Increased peak plasma levels & total plasma exposure w/ combination of itraconazole & gemfibrozil. Increased plasma conc of oral desmopressin. Drugs w/ similar pharmacological properties may potentiate loperamide's effect; drugs that accelerate GI transit may decrease its effect.
