Indications/Uses
Susceptible organisms eg, otitis media, orbital cellulitis, urinary tract, skin & soft tissue, bone & joint infections & post-splenectomy sepsis of unclear etiology.
Dosage/Direction for Use
IV Adult Usual dose: 750 mg to 1.5 g every 8 hr for 5-10 days. Uncomplicated UTI, skin structure infection, disseminated gonococcal infection, uncomplicated pneumonia 750 mg every 8 hr. Severe infection or bacterial meningitis 1.5 g every 6 hr. Max: 3 g every 8 hr. Uncomplicated gonococcal infection 1.5 g IM as single dose w/ oral probenecid 1 g. Ped patient 50-100 mg/kg/day. Bone & joint infection 150 mg/kg/day. Bacterial meningitis 200-240 mg/kg/day in equally divided doses, every 6-8 hr.
Contraindications
Hypersensitivity to cefuroxime or cephalosporins.
Special Precautions
Patients who are hypersensitive to penicillin. Long-term administration may cause development of non-susceptible bacteria. May interfere w/ results of some lab tests, causing false +ve reaction for glycosuria (methods of Benedict, Fehling & Clinitest). False +ve result may occur in direct Coombs test. Concomitant use w/ other nephrotoxic drug (eg, kanamycin, streptomycin, colistin, viomycin, polymyxin, neomycin, gentamycin). Reduce dose according to renal function test in case of pronounced renal insufficiency. Pregnancy. Discontinue breastfeeding or discontinue/abstain from therapy considering the benefit of breastfeeding for the child & the benefit of therapy for the woman. Childn <3 mth.
Adverse Reactions
Glossitis, nausea, vomiting, gastric pyrosis, abdominal pain & diarrhea. Transient & recoverable changes in some lab parameters eg, eosinophilia, leucopenia, & increase of serum transaminase, total bilirubin, & BUN.
Drug Interactions
May affect gut flora, leading to lower estrogen reabsorption & reduced efficacy of combined OCs. Prolonged excretion & elevated peak serum level w/ probenecid. Risk of renal impairment w/ strong-acting diuretics (eg, furosemide) or potential nephrotoxic prep (eg, aminoglycoside antibiotics). Increased INR w/ oral anticoagulants.
