Ibuprofen
Indications/Uses
Fever reduction. Relief of mild to moderate aches & pains eg, dental pain (including teething pain, toothache), headache (including migraine, neuralgia), muscular aches including (sprains & strains, minor arthritis pain, backache), common cold, flu or sore throat, menstrual cramps; other types of pain eg, earache, or pain after vaccination or surgery; inflammation from arthritis, non-serious arthritic conditions, & juvenile RA.
Dosage/Direction for Use
Childn Usually 5-10 mg/kg/dose. All doses to be given every 6-8 hr as needed. Max: 4 doses in 24 hr. Dolan FP susp Childn 7-12 yr 10 mL, 3-6 yr 5-10 mL, 1-2 yr 5 mL. Dolan FP Forte susp Childn 7-12 yr 5 mL, 3-6 yr 2.5-5 mL, 1-2 yr 2.5 mL. Juvenile RA Usually 7.5-12.5 mg/kg/dose every 6 hr.
Administration
May be taken with or without food: May be taken w/ food or milk if stomach upset occurs.
Contraindications
Hypersensitivity to ibuprofen, aspirin or any other NSAIDs. Patients currently taking aspirin or other NSAIDs. Bronchospasm, angioedema, nasal polyps or allergic-type reactions after taking aspirin or other NSAIDs. Stomach ulcers, bleeding or other stomach problems eg, inflammatory bowel disease. Severe heart, liver or kidney (CrCl <30 mL/min) disease. Conditions involving increased tendency for active bleeding. Dehydration due to vomiting, diarrhea or lack of fluids. SLE. Dengue fever. Prior to or after undergoing heart surgery.
Special Precautions
May cause a severe allergic reaction & stomach bleeding. Risk of heart attack or stroke w/ higher doses or prolonged use in childn. Childn w/ heart problems, previous stroke or risk factors (eg, high BP & cholesterol, diabetes, smoker); history of stomach problems eg, heartburn, Crohn's disease or ulcerative colitis; blood clotting disorder (eg, hemophilia); liver or kidney problems; asthma; connective tissue disease; concomitantly taking diuretics; childn 12-18 yr, especially if w/ vol depletion. Discontinue use in case of signs of stomach bleeding; allergic reaction; new symptoms; symptoms do not improve; redness or swelling in the painful area; no relief w/in 24 hr of treatment; pain worsens or lasts >3 days in childn.
Adverse Reactions
Constipation, decreased appetite, diarrhea, dizziness, dyspepsia, otitis media, fever, fluid retention, bloating or flatulence, headache, heartburn, indigestion, itching, stomatitis, nausea, nervousness, rash, tinnitus, pharyngitis, stomach cramps or pain, swelling, vertigo & vomiting.
Drug Interactions
May interfere w/ anti-platelet activity of aspirin limiting its cardioprotective effect. Increased risk of stomach & intestinal bleeding w/ phenylbutazone, indomethacin, salicylates (eg, aspirin) & other NSAIDs (eg, mefenamic acid, naproxen, diclofenac, ketoprofen), including COX-2 inhibitors (eg, celecoxib, etoricoxib); anticoagulants (eg, warfarin, dicumarol) or thrombolytic agents (eg, streptokinase); corticosteroids, SSRIs & anti-platelet agents. Increased toxicity of aminoglycosides (eg, amikacin, gentamicin, neomycin, tobramycin); lithium & MTX. Increased plasma levels w/ antacids eg, Mg hydroxide. Reduced effect of antihypertensives (eg, captopril, enalapril, losartan, telmisartan) & diuretics (eg, furosemide). May worsen heart failure, reduce kidney function & increase blood levels of cardiac glycosides (eg, digoxin). Reduced absorption w/ cholestyramine. Increased risk of kidney damage w/ ciclosporin & tacrolimus. Enhanced risk of bleeding w/ Ginkgo biloba. Altered safety & efficacy w/ medicines affecting liver enzymes. May increase plasma levels of sulfonylureas (eg, gliclazide, glipizide, glimepiride) & insulin. Increased toxicity by reduced metabolism & excretion w/ probenecid. Increased risk of convulsions w/ quinolones. Increased risk of blood problems eg, hemarthrosis & hematoma w/ zidovudine.
