Indications/Uses
HTN & CAD eg, chronic stable angina & angiographically documented CAD in patients w/ heart failure or ejection fraction <40%.
Dosage/Direction for Use
HTN, stable angina, Prinzmetal's angina Initially 5 mg once daily, increased to 10 mg once daily, if necessary. Ped Initially 2.5-5 mg once daily. Elderly patient & those w/ hepatic impairment Lower initial doses may be used.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Possible symptomatic hypotension particularly in patients w/ severe aortic stenosis. Worsening angina & acute MI can develop after starting or increasing the dose, particularly in patients w/ severe obstructive CAD. Amlodipine will not protect against the consequences of abrupt β-blocker w/drawal; gradual β-blocker dose reduction is recommended. Titrate slowly when administering to patients w/ severe hepatic impairment. Safety in pregnancy & lactation has not been established.
Adverse Reactions
Edema, dizziness, flushing, & palpitation. Fatigue, nausea, abdominal pain, somnolence.
Drug Interactions
May produce additive antihypertensive & antianginal effects w/ SL nitroglycerin, long-acting nitrates, β-blockers or other antianginal agents. Increased systemic exposure w/ diltiazem. Plasma conc may be increased w/ strong CYP3A4 inhibitors (eg, ketoconazole, itraconazole, ritonavir). Closely monitor BP when co-administered w/ CYP3A4 inducers.
