Clindamycin HCl
Indications/Uses
GI & oropharyngeal infections caused by Bacteroides fragilis. Resp tract infections, pharyngitis or tonsillitis caused by Strep pyogenes. Liver abscess, actinomycosis, biliary tract infections, staphylococcal bone & joint infections, carrier state of diphtheria, gas gangrene, gynecological infections eg, bacterial vaginosis; necrotizing fasciitis, secondary peritonitis, aspiration pneumonia, pneumonia including lung abscess; septicemia, skin disorders w/ heavy strep or anaerobe colonization. In combination w/ aminoglycosides or metronidazole to treat abdomen & gut wounds, female genital tract infections, endometriosis & pelvic inflammatory disease. Prophylaxis for endocarditis in patients allergic to penicillin, surgical infection in combination w/ other drugs & to prevent perinatal streptococcal infections. Used w/ primaquine in management of pneumocystis pneumonia.
Dosage/Direction for Use
Adult 150-300 mg every 6 hr. Severe infections Increase dose to 300-450 mg every 6 hr. Childn 3-6 mg/kg every 6 hr, <12 mth or weighing ≤10 kg 37.5 mg every 8 hr. Patient w/ high risk of endocarditis prior to surgery where penicillin is inappropriate 600 mg PO 1 hr before procedure. Patient who will undergo procedures w/ general anesth & cannot be given penicillin Injectable 300 mg 10-15 min before the procedure followed by 150 mg PO/IV after 6 hr.
Administration
May be taken with or without food: Swallow whole w/ a full glass of water.
Contraindications
Hypersensitivity to clindamycin & lincomycin. GI disease especially w/ history of colitis. Diarrhea.
Special Precautions
Discontinue treatment when diarrhea or colitis develops. Atopic patients. Monitor blood counts, renal & liver function during prolonged treatment. Renal & hepatic impairment. Pregnancy & lactation. Infants & childn. Elderly.
Adverse Reactions
Abdominal pain or cramps, nausea, vomiting, diarrhea & loose stools w/ traces of blood & mucus. Antibiotic-associated pseudomembranous colitis. Allergic rashes & urticaria, Stevens-Johnson-like syndrome, anaphylaxis, transient leukopenia, leukopenia (w/ agranulocytosis), eosinophilia, thrombocytopenia, erythema multiforme, exfoliative & vesiculobullous dermatitis, polyarthritis, abnormalities in liver function tests, overt jaundice & hepatic damage.
Drug Interactions
Increased risk of resp depression w/ neuromuscular blockers eg, tubocurarine, pancuronium & atracurium. May exacerbate symptoms of antibiotic-associated colitis w/ diphenoxylate, loperamide & other opiate antidiarrheals. Reduced absorption rate w/ kaolin-pectin susp. Antagonism w/ parasympathomimetics eg, neostigmine & pyridostigmine.
