Salbutamol sulfate
Indications/Uses
Syr: Relief of bronchospasm associated w/ reversible obstructive airway diseases eg, bronchial asthma & COPD. Pulmoneb: Prevention & relief of bronchospasm associated w/ reversible obstructive airway disease eg, bronchial asthma; treatment of acute exacerbations of asthma.
Dosage/Direction for Use
Syr Adult & childn >12 yr 5-10 mL (1-2 tsp). Childn 7-12 yr 5 mL (1 tsp), 2-6 yr 2.5-5 mL (1/2 to 1 tsp). To be taken every 6 or 8 hrs. Elderly & patient known to be unusually sensitive to β-adrenergic stimulant drugs Initially 2 mg every 6-8 hrs. Pulmoneb Adult & childn >12 yr 5 mg. Childn 4-12 yr 2.5 mg. To be given every 6-8 hrs via nebulizer.
Administration
Should be taken on an empty stomach: Take 1 hr before or 2 hr after meals.
Contraindications
Hypersensitivity. Syr: Maternal or fetal conditions (eg, eclampsia or severe preeclampsia, intrauterine infection, intrauterine fetal death, antepartum hemorrhage, placenta previa, cord compression, or toxemia of pregnancy); threatened abortion.
Special Precautions
Discontinue use if paradoxical bronchospasm occurs. CV disorders including coronary insufficiency, cardiac arrhythmias or HTN; convulsive disorders; hyperthyroidism; DM; patients unusually responsive to sympathomimetic amines. Metabolic changes (eg, increased blood sugar levels). Acute severe asthma where concomitant therapy w/ steroid, xanthine derivatives, or diuretics, & by hypoxia may result in hypokalemia. Monitor K conc in severe asthma. Pregnancy & lactation. Pulmoneb: Hypersensitivity (eg, urticaria, angioedema, rash, bronchospasm, anaphylaxis, & oropharyngeal edema). Worsening of asthma. Excessive use of sympathomimetic oral inhalations. β2-agonists.
Adverse Reactions
Tremors (particularly the hands), nervousness, dizziness, headache, tachycardia, palpitations, peripheral vasodilation, increased coughing, hoarseness, hypokalemia, hypersensitivity (anaphylaxis, angioedema, bronchospasm, collapse, hypotension, oropharyngeal edema, rash, urticaria), central nervous stimulation, vertigo, angina, paradoxical bronchospasm, cardiac arrhythmias (including atrial fibrillation, supraventricular tachycardia, and extrasystoles). Syr: Conjunctivitis; erythema multiforme, SJS; increased appetite; hyperactivity; central nervous stimulation; mydriasis; myocardial ischemia; epistaxis, hypotension, pallor; epigastric pain, stomachache; feeling of muscle tension, muscle cramps; increased sweating. Pulmoneb: sleeplessness/insomnia, chest pain, HTN, wheezing, asthma exacerbation, bronchitis, nasal congestion, rhinitis, pharyngitis, sinusitis, viral infection, increased hemoptysis in patients w/ pre-existing hemoptysis, otitis media, nausea, vomiting, diarrhea, hypertonia, asthenia, back pain, fever, lymphadenopathy, dry mouth.
Drug Interactions
Other oral or aerosol sympathomimetic bronchodilators, epinephrine; MAOIs, TCAs; β-blockers. Syr: May result in electrocardiographic changes &/or hypokalemia w/ non K-sparing diuretics (eg, loop or thiazide).
