Per 5 mL syr Dextromethorphan HBr 15 mg, phenylpropanolamine HCl 10 mg, chlorpheniramine maleate 2 mg, Na citrate 200 mg

Indications/Uses

Coughs associated w/ colds, flu, bronchitis, pharyngitis, laryngitis & other resp diseases whether allergic or non-allergic in nature.

Dosage/Direction for Use

Adult 5-10 mL every 4 hr. Childn 1-9 yr 2.5-5 mL every 4 hr. Max duration: 1 wk.

Overdosage

Contraindications

Hypersensitivity. Patient w/ renal disease. Dextromethorphan in patient w/ asthma due to release of histamines.

Special Precautions

May cause allergic type reaction (eg, bronchial asthma) in susceptible persons due to FD & C yellow #5 (Tartrazine) content. Antihistamines may cause drowsiness & dulling of mental alertness where loss of attention may lead to accidents. Pregnancy & lactation. Phenylpropanolamine HCl: Caution in patients w/ high BP, toxic goiter, benign prostatic hypertrophy, irregular heart rate, glaucoma, & those taking antidepressants. Heart disease, uncontrolled/untreated high BP.

Adverse Reactions

Drowsiness, abdominal pain, diarrhea, constipation, headache, thickening of bronchial secretions, urinary retention, increased risk of hemorrhagic stroke due to phenylpropanolamine, CNS depression.

Drug Interactions

Dextromethorphan HBr: Decreased metabolism w/ celecoxib, chloramphenicol, vit D. Increased serum levels w/ colchicine, doxorubicin, & phenytoin. Increased serotonergic activities w/ sertraline, toloxatone, alosetron, amphetamine & citalopram. Phenylpropanolamine HCl: Decreased broncho dilatory effects by β-blockers, clozapine. Hypokalemia w/ diuretics. Chlorpheniramine maleate: Co-administration w/ alcohol, antihistamines, CNS depressants & sedatives. Na citrate: Co-administration w/ warfarin, NSAIDs, alteplase, diuretics, barbiturates, buclizine, Ca salts & aspirin.
