Cefixime.
Indications/Uses
Uncomplicated UTI caused by E. coli and Proteus mirabilis; otitis media caused by H. influenzae (positive and negative strain of beta-lactamase), Moraxella catarrhalis and Strep pyogenes; pharyngitis amd tonsillitis caused by S. pyogenes; acute bronchitis and acute exacerbation of chronic bronchitis caused by S. pyogenes, S. pneumoniae and H. influenzae; typhoid fever in paediatric patients with multi-resistant to standard treatment; uncomplicated gonorrhoeae caused by N. gonorrhoeae.
Dosage/Direction for Use
Typhoid fever in children: 10-15 mg/kg body wt/day for 2 weeks.
Patients with CrCl 21-60 mL/min or undergoing haemodialysis: 75% of standard dose, CrCl <20 mL/min or patients undergoing Continuous Ambulatory Peritoneal Dialysis (CAPD): 50% of standard dose.
Capsule: Adults & children >30 kg: 50-100 mg twice daily. May be increased to 200 mg twice daily in severe infections.
Uncomplicated gonorrhea: 400 mg as a single dose.
Dry suspension/oral drops: Children: 1.5-3 mg/kg body weight in 2 divided doses. May be increased to 6 mg/kg body weight in 2 divided doses.
Contraindications
Hypersensitivity.
Special Precautions
Patients with a history of hypersensitivity to penicillin. A history of hypersensitivity, ie. bronchial asthma, skin rash, urticaria. Serious kidney impairment. Patients receiving oral, parenteral nutrition. Elderly. Debilitated patients.
Adverse Reactions
Shock, hypersensitivity (ie. skin rash, urticaria, erythema, pruritus or fever), granulocytopenia, eosinophilia, thrombocytopenia; increased ALT/AST/alkaline phosphatase; acute kidney insufficiency; pseudomembranous colitis, vomiting, diarrhea, abdominal discomfort, abdominal pain, anorexia, nausea, flatulence, constipation; Pulmonary Interstitial Emphysema (PIE syndrome); stomatitis, kandidiasis; vitamin K or vitamin B deficiencies; headache, dizziness; may result in false positive of urine glucose level using Benedict or Fehling solution, or Clinitest; may cause positive result in Direct Coombs test.
Drug Interactions
Concomitant use with anticoagulants, carbamazepine, nifedipine, probenecid, salicylate.
