Povidone-iodine
Indications/Uses
Cream: Major & minor burns, cuts & abrasions. Oint: Protection from infection for burns, scalds, blister, cuts, scratches & grazes. Dry powd spray: Treatment & prevention of infection in wounds, including ulcers, burns, cuts & other minor injuries. Surgical scrub: Pre-op scrubbing by surgical teams. Soln: Cleansing of wounds, lacerations, cuts, burns of various degrees & pre-op antiseptic cleaning of skin & mucous tissues.
Dosage/Direction for Use
Cream Adult, elderly & childn Apply to affected area 1-2 times daily or more if required. Cover affected area w/ dressing or bandage if desired. Oint Adult, elderly & childn Spread evenly over area of damaged skin, 1-2 times a day. Apply dressing if necessary. Dry powd spray Adult, elderly & childn >2 yr Spray from a distance of 6-10 inches onto parts to be treated. Cover w/ dressing if necessary. Apply several times a day (every 3-4 hr). Surgical scrub Adult, elderly & childn Use about 5 mL for hands, otherwise 2 mL/100 cm2 of body surface, & rub in well for 1 min. Add few drops of water to obtain better lather, then rinse off w/ clean water. Soln Apply sufficient amount to affected area at least once.
Contraindications
Hypersensitivity. Patients w/ hyperthyroidism or other thyroid disorders (particularly nodular colloid goitre, endemic goitre & Hashimoto's thyroiditis). Patients taking lithium. Patients using Hg-containing products. Cream/Surgical scrub: Before, during & after radioiodine administration. Childn <1 yr. Oint: Before, during & after radioiodine administration. Childn <2 yr. Dry powd spray: In serous cavities. Prior to or during radioiodine scintigraphy or radioiodine treatment of thyroid carcinoma. Childn <2 yr.
Special Precautions
Special caution when regularly applying to broken skin in patients w/ pre-existing renal insufficiency. Avoid detergent-containing soln if treating open wounds w/ povidone-iodine. Thyroid function tests may be affected. Povidone-iodine contamination of several tests for detecting occult blood in faeces or blood in urine may produce false +ve results. Avoid use during pregnancy & lactation unless potential benefit to mother justifies potential risk to foetus/neonate, or if safer alternative is unavailable. Cream: Patients w/ goitre, thyroid nodules, or other non-acute thyroid diseases are at risk of developing hyperthyroidism from administration of large amounts of iodine. Povidone-iodine should not cover a large area of the skin for a long period (eg, not >10% of total body surface & not >14 days). Monitor thyroid function if necessary. Do not use prior to or after radioiodine scintigraphy or radioiodine treatment of thyroid carcinoma. Do not heat prior to application. Should only be used in childn >1 yr if strictly indicated under medical supervision & its use should be kept to the absolute min. Oint: Povidone-iodine should not cover a large area of the skin for a long period (eg, not >10% of total body surface & not >14 days). Monitor thyroid function if necessary. Thyroid function tests should be performed during prolonged use. Dry powd spray: Avoid inhalation & eye contact. Surgical scrub: Povidone-iodine should not cover a large area of the skin for a long period (eg, not >10% of total body surface & not >14 days). Monitor thyroid function if necessary. Should only be used under medical supervision in childn 1-2 yr.
Adverse Reactions
Transient skin discolouration at application site; local skin reactions; systemic adverse effects (eg, metabolic acidosis, hypernatraemia & renal impairment) if applied to large wounds or severe burns; anaphylactic reactions. Cream: Hyperthyroidism (sometimes w/ symptoms eg, tachycardia or restlessness); contact dermatitis (w/ symptoms eg, erythema, small blisters & pruritus), angioedema. Oint: Iodine-induced hyperthyroidism in patients w/ history of thyroid disease.
Drug Interactions
Concomitant use w/ wound-treatment prep containing enzymatic component leads to weakened effect of both substances. Potential interaction w/ products containing Hg, silver, hydrogen peroxide, & taurolidine. Simultaneous use w/ Hg products may lead to formation of skin-damaging substance. May interfere w/ thyroid function tests or radioiodine therapy. Several tests for detecting occult blood in faeces or blood in urine may be affected (ie, false +ve results) due to the oxidative effect of povidone-iodine prep. Transient dark discolourations at application site when povidone-iodine is used before or after application of octenidine. Povidone-iodine use could lead to transient skin discolouration at application site caused by its own colour. Cream: PVP-iodine complex will react w/ protein & other unsaturated organic compd, leading to impairment of its effectiveness.
