Acute bacterial rhinosinusitis is defined as acute rhinosinusitis caused
by bacteria and is diagnosed based on clinical presentation or presence
of bacteria in the paranasal sinus cavity.
It commonly occurs as a complication of a viral infection (upper respiratory tract infection [URTI]), rhinitis, or other conditions that impair local or systemic immune function.
Streptococcus pneumoniae is the most common cause followed by nontypeable Haemophilus influenzae.
Signs and symptoms are non-specific and typically difficult to differentiate from viral URTI.
Presumptive diagnosis and identification of bacterial versus viral rhinosinusitis may be made when the child has an acute URTI.
Symptomatic treatment is the preferred initial management in children with mild symptoms.
Antibiotic therapy should be initiated as soon as diagnosis has been established. This may help decrease illness duration, relieve symptoms earlier, and prevent recurrences or complications. Antibiotic treatment should be withheld for children with symptoms of <10 days duration.
