Indications/Uses
Bactericidal for both gm +ve/-ve bacteria. Bacterial infections caused by ampicillin-sensitive organisms eg, ear, nose, throat, gynaecological & GI infections, bronchitis, pneumonia, UTI, gonorrhoea, septicaemia, peritonitis, endocarditis, meningitis, enteric fever. Extraperitoneal application to wounds to prevent infection following abdominal surgery.
Dosage/Direction for Use
IM/IV Administer slow inj for 3-4 min. Use direct IV inj only in moderately severe or severe infections over 10-15 min. Adult including elderly Septicaemia, endocarditis, osteomyelitis 500 mg qid to 6x daily for 1-6 wk. Peritonitis, intra-abdominal sepsis 500 mg qid. Local use in abdominal surgery 1 g sterile powd sprinkled into the wound extraperitoneally or into muscle layers to prevent wound infection post-op. Adult Meningitis 2 g every 6 hr IV. Resp tract or skin & skin structure infections 250-500 mg every 6 hr. GI or UTI 500 mg every 6 hr. Septicemia or bacterial meningitis 8-14 g or 150-200 mg/kg daily in equally divided doses every 3-4 hr. Initially given as IV for at least 3 days & continue as IM. Childn Meningitis 150 mg/kg daily IV in divided doses. Childn <10 yr ½ adult routine dose. Childn weighing ≤40 kg Resp tract or skin & skin structure infections 25-50 mg/kg daily in equally divided doses every 6 hr. GI or UTI 50-100 mg/kg daily in equally divided doses every 6 hr. Septicemia or CNS infections 100-200 mg/kg daily in divided doses every 3-4 hr, starting w/ IV for 3 days & continuing w/ IM. Severe renal impairment (CrCl <10 mL/min) May consider dose reduction or dose interval extension. Dialysis patient Administer additional dose after procedure.
Contraindications
Hypersensitivity. History of hypersensitivity to β-lactam antibiotics eg, penicillin & cephalosporins.
Special Precautions
Previous hypersensitivity reactions to β-lactam antibiotics. Serious & occasionally fatal hypersensitivity reactions (anaphylaxis). Patients w/ history of β-lactam hypersensitivity. Not to be given in patients w/ infectious mononucleosis. Avoid use in suspected infectious mononucleosis &/or acute or chronic lymphoid leukaemia. Overgrowth of non-susceptible organisms in prolonged use. Increased risk of developing skin rashes in patients w/ lymphatic leukemia & hyperuricaemia treated w/ allopurinol. Concomitant use w/ OCs. Lactation.
Adverse Reactions
Nausea, vomiting, diarrhoea; moderate & transient increase in transaminases. Skin rash, pruritis & urticaria in patients suffering from infectious mononucleosis & acute or chronic lymphoid leukaemia. Erythema multiforme, SJS, TEN; pseudomembraneous colitis. Supra-infections w/ non-susceptible organism w/ prolonged use.
Drug Interactions
Loss of activity of aminoglycoside. May interfere w/ bacteriostatic drugs. Reduced efficacy of OCs. Increased & prolonged blood levels w/ probenecid. Increased likelihood of allergic skin reactions w/ allopurinol. False +ve readings w/ chemical methods.
