Famotidine
Indications/Uses
Acute duodenal ulcer; active benign gastric ulcer; GERD including GERD-associated esophagitis; pathologic GI hypersecretory conditions eg, Zollinger-Ellison syndrome. Prophylaxis of recurrent duodenal ulcer.
Dosage/Direction for Use
Duodenal ulcer Acute therapy: 40 mg at bedtime or 20 mg bid for 4-8 wk. Maintenance therapy: 20 mg at bedtime for 1 yr or as clinically indicated. Benign gastric ulcer 40 mg at bedtime for 6-8 wk. GERD 20 mg bid for up to 6 wk. Esophagitis associated w/ GERD 20-40 mg bid for up to 12 wk. Pathologic GI hypersecretory conditions Initial adult dose: 20 mg every 6 hr, may increase up to max of 160 mg every 6 hr. Adult w/ moderate & severe renal insufficiency 50% of normal dose every 36-48 hr.
Administration
May be taken with or without food.
Contraindications
History of hypersensitivity to famotidine or other H2-receptor antagonist.
Special Precautions
Consider dose reduction or longer intervals in patients w/ moderate (CrCl <50 mL/min) to severe (CrCl <10 mL/min) renal impairment. Pregnancy. Discontinue treatment or breastfeeding during lactation. Childn. Elderly w/ impaired renal function.
Adverse Reactions
Headache, dizziness; constipation, diarrhea. SJS, TEN.
Drug Interactions
Reduced systemic exposure to ketoconazole. Decreased GI absorption & plasma conc of itraconazole. Increased conc w/ probenecid. Reduced efficacy of atazanavir.
