Biapenem
Indications/Uses
Sepsis, pneumonia, lung abscess, secondary infections in chronic resp lesion, complicated cystitis, pyelonephritis, peritonitis, parametritis due to biapenem-susceptible strains of Staph sp, Strep sp, Strep pneumoniae, Enterococcus sp (excluding Enterococcus faecium), Moraxella sp, E. coli, Citrobacter sp, Klebsiella sp, Enterobacter sp, Serratia sp, Proteus sp, H. influenzae, Pseudomonas aeruginosa, Acinetobacter sp, Peptostreptococcus sp, Bacteroides sp, Prevotella sp & Fusobacterium sp.
Dosage/Direction for Use
IV drip infusion Adult 1.2 g daily (potency) in 2 divided doses over 30-60 min period. May be adjusted according to patient's age & symptoms. Max: 1.2 g daily (potency).
Contraindications
History of shock due to any of the excipients of Omegacin. Concomitant use w/ Na valproate.
Special Precautions
History of hypersensitivity to carbapenem, penicillin or cephem antibiotics. Personal or familial predisposition to allergic reactions eg, bronchial asthma, rash or urticaria; convulsion or disturbed consciousness. Patients w/ poor oral food intake & general health or those receiving parenteral alimentation; CNS disorder or history of epilepsy. Carefully observe for development of vit K deficiency; immediately after beginning of administration. Obtain any history of anaphylactoid reactions to drugs or allergy to antibiotics or other agents. False +ve results in urine glucose test w/ Benedict's & Fehling's soln & Clinitest. +ve results in direct Coombs' test. Reduced potency when mixed w/ amino-acid prep containing L-cysteine or L-cystine. Severe renal disorder. Pregnancy. Discontinue breastfeeding during treatment. Low birth wt infants, newborns, suckling infants, infants & childn. Elderly (reduced physiological function & w/ development of bleeding tendency due to vit K deficiency).
Adverse Reactions
Increased ALT (GPT); increased serum amylase. Rash, pruritus; eosinophilia, prolonged prothrombin time, basophilia, thrombocytosis, erythropenia, decreased hematocrit value & Hb, neutrophilia, lymphocytosis, monocytosis; increased AST (GOT), γ-glutamyl transferase, alkaline phosphatase, leucine aminopeptidase, LDH & bilirubin; increased N-acetyl-β-D-glucosaminidase, urinary β2 microglobulin & BUN, proteinuria; diarrhea, nausea, abdominal pain; hyperkalemia, fever, headache. SJS/TEN.
Drug Interactions
Decreased valproate blood levels & recurrent epileptic seizures w/ Na valproate.
