Initial: Oral, 2 to 5 mg per day, the dosage being adjusted according to PT/ INR determinations.
Maintenance: Oral 2 to 10 mg per day, as indicated by PT/INR determinations.
Duration of therapy: Individualize the duration of therapy. Continue anticoagulant therapy until the danger of thrombosis and embolism has passed.
