Midodrine HCl
Indications/Uses
Symptomatic orthostatic hypotension due to autonomic dysfunction when corrective factors have been ruled out & other forms of treatment are inadequate.
Dosage/Direction for Use
Initially 2.5 mg bid-tid, may be increased wkly up to 10 mg tid.
Administration
Should be taken with food: The last daily dose should be taken at least 4 hr before bedtime to prevent supine HTN.
Contraindications
Hypersensitivity. HTN, bradycardia, urinary retention, ischaemic heart disease, CHF, cardiac conduction disturbances, narrow angle glaucoma, serious obliterative blood vessel disease, cerebrovascular occlusions & vessel spasms, aortic aneurism, proliferative diabetic retinopathy, thyrotoxicosis, pheochromocytoma, severe renal insufficiency (CrCl <30 mL/min) & serious prostate disorder.
Special Precautions
Renal & hepatic impairment. Patients w/ initial supine systolic pressure >180 mmHg, urinary retention problems, orthostatic hypotensive diabetics, & those w/ visual problems who are also taking fludrocortisone. Monitor supine & sitting BP in maintenance therapy. Evaluate potential supine & sitting HTN at initiation of treatment. Concomitant use w/ cardiac glycosides (eg, digitalis), psychopharmacologic agents, β-blockers or other agents that directly or indirectly reduce heart rate, fludrocortisone acetate. Discontinue & re-evaluate if bradycardia occurs. Pregnancy & lactation.  
Adverse Reactions
Chills & pilo-erection; paresthesia & pruritus, supine HTN. Urinary retention, headache.
Drug Interactions
Increased BP w/ reserpine, guanethidine, TCAs, antihistamines, thyroid hormones & MAOIs. Effect may be blocked by α-adrenergic blockers eg, prazosine & doxazosine. Heart block may occur w/ digitalis. May potentiate hypertensive effects of fludrocortisone acetate. Drugs that directly or indirectly reduce heart rate.
