Glimepiride

Indications/Uses

Management of type 2 DM (NIDDM) as an adjunct to diet & exercise to lower blood glucose. In combination w/ metformin or insulin in patients whose hyperglycemia cannot be controlled by diet & exercise in conjunction w/ a single oral hypoglycemic agent.

Dosage/Direction for Use

Adult Initially 1-2 mg once daily. Maintenance dose: 1-4 mg once daily. After a dose of 2 mg once daily, may increase in increments of 2 mg at 1-2 wk interval. Max dose: 8 mg once daily. Childn 10-18 yr Initially 1 mg once daily. Maintenance: 1-4 mg once daily. Elderly Initially 1 mg/day. Combination w/ insulin therapy w/ fasting glucose level of >150 mg/dL Initially 8 mg once daily. Renal impairment w/ CrCl <22 mL/min Initially 1 mg then, dose increments based on fasting blood glucose.

Overdosage

View Losu overdosage for action to be taken in the event of an overdose.

Administration

Should be taken with food: Take immediately before or during breakfast, or the 1st main meal of the day. Do not skip meals.

Contraindications

Hypersensitivity to glimepiride or sulfonamides. Diabetic ketoacidosis (w/ or w/o coma).

Special Precautions

Hypoglycemia may occur. Impaired renal or hepatic function, malnourished patients, elderly. Avoid use w/ sulfonamides. Discontinue therapy & administer insulin if patient is exposed to stress (fever, trauma, infection, surgery). Pregnancy (insulin is recommended) & lactation.

Dizziness, headache; hypoglycemia; nausea; weakness.

Drug Interactions

Increased effect/toxicity w/ delavirdine, ketoconazole, nicardipine, NSAIDs, sulfonamides, tolbutamide. Increased hypoglycemic effect w/ β-blockers, chloramphenicol, cimetidine, fibric acid derivatives, fluconazole, pegvisomant, salicylates, sulfonamides, TCAs. Increase effect of cyclosporin. May induce disulfiram-like reaction w/ ethanol. Decreased levels/effect w/ carbamazepine, phenobarb, phenytoin, rifampin, rifapentine, secobarbital. Decreased effect w/ corticosteroids, estrogens, OCs, thiazide & other diuretics, phenothiazines, NSAIDs, thyroid products, nicotinic acid, INH, sympathomimetics, urinary alkalinizers & charcoal. May cause hypoglycemia w/ ethanol, chromium, garlic & gymnema.
