Indications/Uses

HTN. Renal protection in type 2 diabetic patients w/ proteinuria. Reduction in the risk of CV morbidity & mortality rate in hypertensive patients w/ left ventricular hypertrophy.

Dosage/Direction for Use

Adult HTN Monotherapy: 50 mg once daily. Maintenance dose: Total dose range 25-100 mg once daily to bid. Combination therapy 50-100 mg once daily w/ 12.5 mg hydrochlorothiazide. May be increased to 100 mg once daily w/ 25 mg hydrochlorothiazide if BP is not desirely controlled after 3 wk (or after 2-4 wk in severe HTN). Renal protection in type 2 diabetes patients w/ proteinuria Initially 50 mg once daily. May be increased to 100 mg once daily based on BP response. Reduction in the risk of CV morbidity & mortality rate in hypertensive patients w/ left ventricular hypertrophy Initially 50 mg once daily; low dose hydrochlorothiazide may be added. May be increased to 100 mg once daily based on BP response. Ped patients ≥6 yr weighing >50 kg 50 mg once daily. May be increased to 100 mg once daily, ≥20 to <50 kg 25 mg once daily. May increased to max of 50 mg once daily. Patients w/ intravascular vol depletion (eg, treated w/ high-dose diuretics) HTN Monotherapy: Initially 25 mg once daily.

Special Precautions

Intravascular vol depletion, preexisting renal insufficiency, hepatic impairment. Concomitant use w/ K supplement or K sparing diuretics. Discontinue if angioedema of the face, lips, tongue, glottis, larynx occurs. Pregnancy is contraindicated. Lactation. Ped patients <6 yr.

Adverse Reactions

Headache, dizziness. Asthenia/fatigue, edema-swelling, abdominal & chest pain, diarrhea, dyspepsia, nausea, insomnia, pharyngitis, swelling of the face, lips, pharynx &/or tongue, angioedema including swelling of the larynx & glottis; causing airway obstruction, cough, fever, orthostatic effects, hypersensitivity, anaphylactic reactions, hepatitis, liver function abnormalities, vomiting, anemia, thrombocytopenia, musculoskeletal pain, arthralgia, palpitation, tachycardia, back pain, cramps, migraine, dysgeusia, urticaria, rash, vasculitis, including Henoch-Schonlein purpura.

Drug Interactions

Increased serum K may occur w/ K-sparing diuretics (eg, spironolactone, triamterene, amiloride), K supplements or salt substitutes containing K. Decreased antihypertensive effect by indomethacin, NSAIDs. Reduced levels of active metabolites w/ rifampin & fluconazole. Lithium. May result further renal function deterioration w/ NSAIDs including selective COX-2 inhibitors.
