Fosfomycin
Indications/Uses
Susceptible infections eg, acute bacterial cystitis & urethra-vesical syndrome, acute episodes of relapsing bacterial cystitis, nonspecific bacterial urethritis. E. coli. Asymptomatic bacteriuria in pregnancy.
Dosage/Direction for Use
Women & female adolescent >12 yr Acute uncomplicated cystitis 1 sachet once daily. Prophylaxis for peri-op transrectal prostate biopsy 1 sachet 3 hr prior to procedure & 1 sachet 24 hr after the procedure.
Administration
Should be taken on an empty stomach: Dissolve in a glass of water & take immediately. For acute uncomplicated cystitis in women & female adolescents it should be taken on an empty stomach preferably before bedtime & after emptying the bladder. Take 2-3 hr before or after a meal.
Contraindications
Hypersensitivity. Patients w/ renal impairment (CrCl <10 mL/min).
Special Precautions
Not to be used in patients w/ fructose intolerance, glucose-galactose malabsorption or sucrase-isomaltase insufficiency. Persistent infection & male patients w/ UTI. Should only be used during pregnancy if clearly necessary. Single dose of oral fosfomycin can be used during breast-feeding. Childn <12 yr.
Adverse Reactions
Vulvovaginitis; headache, dizziness; diarrhea, nausea, dyspepsia, abdominal pain.
Drug Interactions
Reduced oral absorption w/ metoclopramide. Absorption may be delayed w/ food. Increased anticoagulant activity in patients receiving antibiotics therapy eg, fluoroquinolones, macrolides, cyclins, clotrimozaxole & certain cephalosporins.
