Phenytoin Na
Indications/Uses
Cap: Psychomotor epilepsy, migraine & trigeminal neuralgia. Inj: Control of status epilepticus (grand mal type).
Dosage/Direction for Use
Cap Adult 100 mg tid, increase to 200 mg tid if necessary. Childn 30 mg bid. Inj 100-250 mg by slow IV, then 100-150 mg 30 min later. Max IV rate: 50 mg/min.
Administration
Should be taken with food: When administering to patients on nasogastric or other enteral feeds, do not administer feeds 2 hr before or after a dose. Be consistent throughout therapy in relation to feed times. Swallow cap whole, do not open/chew/crush. Do not switch dosage forms/brands w/o prior consideration.
Contraindications
Pregnancy.
Special Precautions
Not for seizures due to hypoglycemia. Impaired liver function, slow acetylators. SA node depression & AV block (on IV administration). Avoid abrupt w/drawal; lactation. Elderly & gravely ill patients.
Adverse Reactions
Nystagmus, ataxia, slurred speech, confusion, dizziness, gingival hyperplasia, hirsutism, morbilliform rash, rickets, osteomalacia, lupus erythematosus syndrome, leukopenia, thrombocytopenia, pancytopenia, granulocytopenia. Inj: CV collapse & CNS depression. Hypotension (if administered rapidly). Cardiac arrhythmia, ventricular fibrillation & ventricular arrest.
Drug Interactions
Enhanced metabolism by barbiturates. Inhibited metabolism by chloramphenicol, coumarin anticoagulants, disulfiram, phenylbutazone, phenyramidol, INH, sulfaphenazole, sulthiame, valproic acid, cimetidine & sulfonamides.
