Indications/Uses
Alzheimer's disease (moderate to severe, w/ or w/o dementia) & Parkinson's disease w/ dementia.

Dosage/Direction for Use
Initially 5 mg daily in the morning for 1st wk, increased in wkly increments of 5 mg to max dose 20 mg daily. Dose ≥10 mg daily should be taken in 2 divided doses. Patient w/ moderate renal impairment (CrCl 40-60 mL/min/1.73 m2) Max: Reduce dose to 10 mg daily. Patient w/ severe renal impairment (CrCl 5-29 mL/min) Target dose: 10 mg.

Overdosage

May be taken with or without food.

Special Precautions
Closely monitor patients w/ recent MI, uncompensated CHF & uncontrolled HTN; conditions that increase urinary pH (eg, drastic changes in diet, renal tubular acidosis or severe UTI). Seizures have occurred rarely; caution in patients at risk of convulsions. Patients w/ renal impairment. Should be used during pregnancy only if potential benefit justifies potential risk to fetus. Caution when administered to nursing mother. No adequate & well-controlled trials documenting the safety & efficacy in any illness occurring in childn.

Adverse Reactions
Constipation, dizziness, headache & somnolence. Anxiety, hallucinations, confusion, abnormal gait, hypertonia, vomiting, cystitis & increased libido.

Drug Interactions
May increase both incidence & severity of adverse effects w/ other NMDA antagonists eg, amantadine, ketamine or dextromethorphan. May enhance effects of dopaminergics & antimuscarinics. May reduce actions of barbiturates & antipsychotics. May alter effects of antispasmodics, baclofen & dantrolene. Reduced clearance under alkaline urine conditions; caution w/ drugs eg, carbonic anhydrase inhibitors & Na bicarbonate.
