Indications/Uses
Dosage/Direction for Use
Adult 10 mg once daily, may be increased to 20 mg.
Administration
Should be taken on an empty stomach: Take at least 15 min before meals.
Contraindications
Hypersensitivity. Left-ventricular outflow tract obstruction; untreated CHF; unstable angina pectoris or recent (w/in 1 mth) MI. Severe hepatic or renal (GFR <30 mL/min) impairment, including patients undergoing dialysis. Co-administration w/ strong CYP3A4 inhibitors, cyclosporin, grapefruit/grapefruit juice.
Special Precautions
Patients w/ sick sinus syndrome (w/o a pacemaker), left ventricular dysfunction, ischaemic heart disease. Development of cloudy peritoneal effluent in patients on peritoneal dialysis. Concomitant use w/ CYP3A4 inducers eg, anticonvulsants (eg, phenytoin, carbamazepine) & rifampicin; alcohol. Galactose intolerance, total lactase deficiency or glucose-galactose malabsorption. Minor influence on the ability to drive & use machines. Mild to moderate renal impairment. May enhance effect in patients w/ moderate hepatic impairment. Not recommended in women of childbearing potential not using contraception; during pregnancy & lactation. Childn <18 yr. Elderly.
Adverse Reactions
Headache; tachycardia, palpitations; flushing; peripheral oedema.
Drug Interactions
Increased plasma levels w/ strong CYP3A4 inhibitors (eg, ketoconazole, itraconazole, ritonavir, erythromycin, troleandomycin, clarithromycin); cyclosporin. Inhibits metabolism w/ grapefruit or grapefruit juice. Reduced antihypertensive effect w/ CYP3A4 inducers eg, anticonvulsants (eg, phenytoin, phenobarb, carbamazepine) & rifampicin. Potentiated effect w/ alcohol. Increased absorption w/ midazolam. Reduced bioavailability w/ metoprolol. Mean increase in digoxin Cmax. May increase bioavailability & hypotensive effect w/ higher dose of cimetidine. Increased hypotensive effects w/ other medications affecting BP eg, α-blockers, TCAs, neuroleptics. Reduced hypotensive effect w/ corticosteroids. Terfenadine, astemizole, class III anti-arrhythmics (eg, amiodarone, quinidine, sotalol).
