Anxiety; depression; mixed anxiety-depression; anxiety, mixed anxiety-depression, or depression associated w/ other functional or organic disease; panic disorders.

Xanor Individualized dosage. Anxiety Usual starting dose: 0.75-1.5 mg daily in divided doses. Usual dose range: 0.5-4 mg daily in divided doses. Depression Usual starting dose: 1.5 mg daily in divided doses. Usual dose range: 1.5-4.5 mg daily in divided doses. Panic disorders Usual starting dose: 0.5-1 mg at bedtime or 0.5 mg tid. Usual dose range: Adjust dose in increments no >1 mg/day every 3-4 days. Additional doses can be added until a schedule of tid or qid is achieved. Elderly Usual starting dose: 0.5-0.75 mg daily in divided doses. Usual dose range: 0.5-0.75 mg/day in divided doses, may be gradually increased if needed & tolerated. Xanor XR Anxiety Usual starting dose: 1 mg daily in 1 or 2 doses. Usual dose range: 0.5-4 mg daily in 1 or 2 doses. Depression Usual starting dose: 1 mg daily in 1 or 2 doses. Usual dose range: 0.5-4.5 mg daily in 1 or 2 doses. Panic disorders Usual starting dose: 0.5-1 mg at bedtime or 0.5 mg bid. Usual dose range: Adjust dose in increments no >1 mg/day every 3-4 days. Elderly Usual starting dose: 0.5-1 mg daily in 1 or 2 doses. Usual dose range: 0.5-1 mg/day, may be gradually increased if needed & tolerated.

May be taken with or without food: Side effects eg sleepiness/drowsiness may be reduced iftaken immediately after meals. Xanor XR Take in the morning if to be given once daily. Do not chew/crush/break.

Hypersensitivity to alprazolam, or to benzodiazepines.

Habituation & emotional/physical dependence may occur. Increased risk of dependence w/ higher doses & long-term use, & further in patients w/ history of alcoholism or drug abuse. W/drawal symptoms following rapid decrease or abrupt discontinuation. Caution when using higher doses in treating panic disorders in depressed patients or those in whom there is reason to expect concealed suicidal ideation or plans. Reports of episodes of hypomania & mania in patients w/ depression. Use has not been established in depression w/ psychotic features, in bipolar disorders, or in endogenous depression. Concomitant use w/ opioids. Caution while operating motor vehicles or engaging in other dangerous activities. Caution in patients w/ impaired renal or hepatic function. Apprise patient of the potential hazard to the fetus when used during pregnancy or they become pregnant during treatment. Do not breastfeed during treatment. Safety & efficacy have not been established in childn.

Depression; sedation, somnolence, ataxia, memory impairment, dysarthria, dizziness, headache; constipation, dry mouth; fatigue, irritability. Decreased appetite; confusional state, disorientation, decreased libido, anxiety, insomnia, nervousness, increased libido; balance disorder, abnormal coordination, disturbance in attention, hypersomnia, lethargy, tremor; blurred vision; nausea; dermatitis; sexual dysfunction; decreased/increased wt.

Additive CNS depressant effect including resp depression w/ opioids, alcohol or other drugs producing CNS depression. Conc may be increased & activity enhanced w/ CYP3A4 inhibitors. Not recommended in co-administration w/ ketoconazole, itraconazole or other azole-type antifungals. Consider dose reduction in co-administration w/ nefazodone, fluvoxamine, & cimetidine. Caution w/ fluoxetine, propoxyphene, OCs, diltiazem, or macrolide antibiotics eg, erythromycin, & troleandomycin. Large impairment of clearance, prolonged elimination t_1/2 & enhanced clinical effects w/ low doses of ritonavir (HIV PI). Increased digoxin conc especially in elderly >65 yr.
