Co-amoxiclav (amoxicillin & clavulanic acid). Amoxicillin 500 mg, clavulanic acid 125 mg
Indications/Uses
Acute bacterial sinusitis, otitis media & exacerbations of chronic bronchitis; community acquired pneumonia, cystitis, pyelonephritis; skin & soft tissue infections particularly cellulitis, animal bites, severe dental abscess w/ spreading cellulitis; bone & joint infections particularly osteomyelitis.
Dosage/Direction for Use
Adult & childn ≥40 kg One 500 mg/125 mg dose tid. Childn <40 kg 20 mg/5 mg/kg daily to 60 mg/15 mg/kg daily in 3 divided doses. Renal impairment Adult & childn ≥40 kg w/ CrCl 10-30 mL/min 500 mg/125 mg bid, <10 mL/min 500 mg/125 mg once daily, hemodialysis 500 mg/125 mg every 24 hr, plus 500 mg/125 mg during dialysis, to be repeated at the end of dialysis. Childn <40 kg w/ CrCl 10-30 mL/min 15 mg/3.75 mg/kg bid (max 500 mg/125 mg bid), <10 mL/min 15 mg/3.75 mg/kg as a single daily dose (max 500 mg/125 mg), hemodialysis 15 mg/3.75 mg/kg once daily. Administer 15 mg/3.75 mg/kg prior to hemodialysis, then another 15 mg/3.75 mg/kg after hemodialysis in order to restore circulating drug levels.
Administration
Should be taken with food: Take w/ meal to minimise potential GI intolerance.
Contraindications
Hypersensitivity to amoxicillin, clavulanic acid or any of penicillins. History of severe immediate hypersensitivity reaction (eg, anaphylaxis) to another β-lactam agent (eg, cephalosporin, carbapenem or monobactam). History of jaundice/hepatic impairment due to amoxicillin/clavulanic acid.
Special Precautions
Monitor hepatic function at regular intervals. Carefully monitor prothrombin time or INR w/ addition or w/drawal of amoxicillin if co-administration w/ oral anticoagulants is necessary. Perform close clinical monitoring during combination w/ mycophenolate mofetil & shortly after antibiotic treatment. Avoid use during pregnancy unless use is considered essential. Should only be used during breast-feeding after benefit/risk assessment. Childn <2 yr.
Adverse Reactions
Diarrhoea, nausea & vomiting. Mucocutaneous candidosis.
Drug Interactions
Reports of reduced predose conc of mycophenolate mofetil active metabolite (mycophenolic acid). Amoxicillin: Increased INR w/ acenocoumarol or warfarin. May reduce excretion of MTX causing potential increase in toxicity. Decreased renal tubular secretion that may result in increased & prolonged blood levels w/ probenecid.
