Indications/Uses
Secondary treatment of acute hypotension or shock according to different diseases (shock due to MI, & septicemia caused by shock, anaphylactic shock, hypotension or shock, according to the decreased circulating blood vol, acute hypotension during anesth) & cardiac arrest.
Dosage/Direction for Use
Acute hypotension or shock Individualized dose. IV infusion at 2-3 mL/min. Maintenance: 0.5-1 mL/min. Cardiac arrest Inj IV in case of acute hypotension or shock after retaining valid heart rate & ventilation while cardiac resuscitation.
Contraindications
Patients w/ low BP due to lack of blood vol (except for emergency situations to maintain cerebral perfusion); high BP; hyperthyroidism; pheochromocytoma; pulmonary heart disease; severe arrhythmia; anesthetized fingers, toes, nose. Renal dysfunction. Pregnancy.
Special Precautions
Hypersensitivity w/ Na sulfite. Patients w/ cocaine poisoning; ventricular tachycardia; atherosclerosis; heart disease; bradycardia; coronary, mesenteric, peripheral vascular thrombosis; hypotension due to MI; Prinzmetal angina. Do not use as substitute for blood or fluids. May cause an excessive increase in BP & acute pulmonary edema action, arrhythmia, cardiac arrest may occur. Frequently check BP & administration rate to avoid high increase in BP. May cause remarkable peripheral & internal vasoconstriction, decrease in renal perfusion & urea emission, decrease in blood vol despite normal BP, tissue hypoxia, metabolic acidosis w/ continuous use w/o replacement for maintaining of BP; consider using w/ blood vol replacement. Lactation. Elderly.
Adverse Reactions
Ischemic injury due to potent vasoconstrictor action & tissue hypoxia; cardiogenic shock, arrhythmia, bradycardia, stress cardiomyopathy; anxiety, transient headache; resp difficulty; extravasation necrosis at inj site.
Drug Interactions
Increased risk of ventricular tachycardia or fibrillation w/ cyclopropane & halothane anesth. May cause severe, prolonged HTN w/ MAOIs or antidepressants of triptyline or imipramine types.
